Page 1 of Page 1 of Page 1 of Page 1 of COMBINED SYNOPSIS/SOLICITATION RFQ# 36C24822Q1676 ACREDITATION CONSULTING SERVICE 08/19/2022 The Veterans Administration requires Accreditation Readiness Consulting Service in support of the Veteran Integrated Service Network (VISN) 8, to include all eight (8) VISN 8 facilities. The Contractor shall provide all labor, materials, management, supplies, transportation, equipment, reports, and anything else necessary to successfully furnish all requested Accreditation Readiness Consulting Service as identified in the Performance Work Statement (PWS), dated 08/17/2022. The Contractor shall perform and deliver Mock Survey Services. A Mock The Joint Commission (TJC) survey and/or Long-Term Care Institute (LTCI) shall be conducted at each VISN 8 facility to assist in the continuous process improvement and readiness efforts. The Mock Survey process shall be conducted in accordance with The Joint Commission, Survey Activity Guide for Health Care Organizations All Accreditation Programs, dated January 2021 or most current version. These surveys shall focus on frequently cited standards, problematic standards, the high-risk areas of, but not limited to, sterile compounding, sterile processing, suicide, hemodialysis, environment of care, life safety and any topics pertinent and chosen by the facility. The survey process shall identify important organizational, and systems issues related to safety, quality of care, treatment and services and interfaces between such processes. Each survey shall be a multi-day survey. Each facility will be allowed 1 consultative visit (up to 4 consecutive days) that will not exceed 12 consultant days per site. Each consultant team for the multi-day surveys shall have 4 consultant surveyors. The combination and expertise of the team will be at the discretion of the facility. Each facility can choose the desired combination of team members to consist of Nurse, physician, and LS/EOC. Each consultant shall have a minimum of three years of surveying experience. Each of the consultants shall have experience, education, and knowledge of infection control processes.
