The Eye Clinic Section of the newly constructed leased consolidation project at New Port Richey Community Based Outpatient Clinic (NPR CBOC) at 7932 Little Road, New Port Richey, FL 34654-3419. The facility is combining and expanding its services. The NPR CBOC is expanding services in the new facility and has requirements for additional diagnostic and treatment equipment. The procurement of the below listed equipment is required for the expansion and the replacement of existing equipment that has reached its life expectancy. The Eye Lanes will be utilized in the eye clinic for routine and emergent exams completed by the NPR CBOC and local providers. The contractor shall provide all resources (labor, tools, software) necessary to accomplish the deliverables described in this statement of Need (SON). The vendor shall deliver and install all equipment listed below. All items must meet all the salient characteristics identified below. Installation must meet manufacturer's specifications. Since the systems requested are currently being used and are approved for use in the existing and surrounding James A. Haley CBOCs (Community Based Outpatient Clinics) this is a Brand Name Only request. This ensures continuity of care enterprise wide, no need to stock consumable supplies that are not currently being used, and no requirement for additional clinical staff and maintenance staff training.
