The Department of Veterans Affairs, Network Contracting Office 8 San Juan, Puerto Rico is currently conducting a market survey for potential small business qualified firms capable of providing the necessary personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, and other items and non-personal services. Necessary to perform the required service Integrated Pest Management Services for the VA Caribbean Healthcare System (VACHS) facility located in San Juan with possibility of additions/deletions via supplemental agreement on a case-by-case basis for others VACHS clinics in Puerto Rico. The North American Industry Classification System (NAICS) is 561710 Exterminating and Pest Control Services, and Product Service Code (PSC) is S207 Housekeeping- Insect/Rodent Control. Responses to this notice shall include company/individual name, size status for the above reference NAICS code, a service capability statemen, Unique Entity Identifier (UEI) number, address, point of contact and examples of similar facilities to which similar services have been provided. Contractors should provide information on whether they are certified SDVOSB, VOSB, Hub zone, 8(a), women-owned concern. Specific areas to be addressed in capability packages submitted by respondents are as follows: 1. Capability Statements, if considering sub-contracting state so in your response and include sub-contractor information to include UEI and name. 2. Provide certification as Commercial Pesticide Applicators in the category of Industrial, Institutional, Structural, and Health Related Pest Control. NOTE: In Puerto Rico certification shall mean a current Certificate of Competency in Pest Management in any and all categories or sub-categories as identified by the Commonwealth of Puerto Rico under the contract. All other definitions are as defined in CFR 40 subchapter E, parts 150-189. (Last amended 04/08/2022). Contractor shall hold certification throughout the term of the contract 3. Provide certification on pest control and shall be fully certified in all pest management categories in which work is to be performed. Contractor shall have a current certification on pest control categories issued by the Commonwealth of Puerto Rico Department of Agriculture. The Contractor shall be licensed by the Commonwealth of Puerto Rico as a pesticide application business (see Act No. 49 of June 10, 1953, Pesticide Act of Puerto Rico, as amended). Contractor shall submit proof of licensing. 4. Identify any other companies owned (wholly or in part) by the owners of this business who provide goods or services that are registered under the same or a related NAICS code with the Center for Veterans Enterprise (CVE); Provide the certification type (SDVOSB/VOSB), Federal Identification Number, and state of incorporation for each. 5.
