STATEMENT OF WORK Fisher House Bathroom Refurbishment West Palm Beach VA Medical Center 548-22-2-5853-0066 INTRODUCTION The Fisher House is a lodging facility on the grounds of the VA Medical Center for families of our Veterans who are hospitalized. The home like atmosphere is to provide a tranquil retreat of respite for families while their loved one is hospitalized. The Fisher House s current 9 bathrooms need to have all lighting, shelves and mirrors replaced due to pitting and rust. Fisher House will supply needed lighting, shelves and mirrors and contractor will supply all other materials including paint. GENERAL DESCRIPTION Contractor will remove existing shelves, vanity lighting and mirrors. Contractor will install new 9 mirrors, 8 shelves and 18 vanity lights with those supplied. Contractor will paint the existing trim which includes the wainscotting trim and interior side of doors with a bright white in satin finish. Contractor will paint the existing walls using Benjamin Moore Apparition 860 Egg shell finish Contractor will paint the vanity in room 1 & 3. The vanity is 62 X 25 . It will be painted with an epoxy paint. The color is Benjamin Moore Rockport Gray HC-105. Contractor does not supply the new shelving, lights, or mirrors. Contractor will supply all other materials including paint, spackling, and any other materials to complete the job. Contractor is responsible for the clean-up during and after the project is complete. Contractor will patch all existing holes not utilized and caulk where necessary. SAFETY The Contractor will be required to verify that safety equipment of OSHA approved and that the employees have been trained on the proper use of this equipment. The Contractor will be required to follow all safety policies as set forth by OSHA in reference to this type of work. The Contractor will be required to provide the above information as well as a safety plan in performing this work. Failure to provide this information will not be reason for additional time or delay claim for the Contract. Temporary construction partitions must meet the guidelines set forth in the ICRA specific to the Contract. PHASING PLAN No phasing. Fisher House will be closed during the 21 days to complete the job. PROJECT PERFORMANCE TIME The total contractor performance period shall be 21 calendar days for completion. The project will not start until after May 15th. Contractor will have access to the Fisher House 7 days a week/ 24 hours. Guests will be placed in hotel while the project is going on. After work begins on the site, the Contractor shall work continuously during approved work hours. LOCATION & ACCESS Building #4, Fisher House West Palm Beach VA Medical Center 7305 N Military Trail West Palm Beach, FL 33410 POC/COR: Shelley Prickett Phone: 561-422-5554 Email: shelley.prickett2@va.gov WORK HOURS Fisher House will be closed during this project and access to the Fisher House will be available 24/7 COORDINATION Shutdown of utilities must be coordinated with the COR with a minimum of two (2) weeks notice. RECEIPT OF MATERIALS Address all shipment of materials and supplies to the Contractor not the Government. The Contractor must be on hand to accept shipments; the Government will not accept deliveries on the behalf of the Contractor. SPECIFIC ENVIRONMENTAL REQUIREMENTS The Contractor is responsible for ensuring that materials and supplies are in accordance with VA policy for no or low Volatile Organic Compounds (VOC). WARRANTIES The Contractor will provide a one-year warranty covering all material and labor upon approved final inspection. PROTECTION OF EXISTING PROPERTY The Contractor shall be responsible for restoring the area to its original condition at the completion of the work. The Contractor shall be responsible for repair of any damage incurred to buildings or property as a result of construction activities. CLEAN-UP AND DISPOSAL The Contractor shall be responsible for the removal and disposal of all trash/debris connected with construction activities on a daily basis. The contractor is responsible for necessary dumpsters. Recyclable products shall be disposed of in accordance with applicable statutes, in respective containers.
