Page 8 of 8 COMBINED SYNOPSIS/SOLICITATION The West Palm Beach VA Medical Center located at 7305 N Military Trail, West Palm Beach, FL 33410 has a requirement to provide Five Year Standpipe inspection and FDC Check Valve Testing on Fire Sprinkler System. The attached Statement of Work (SOW) shall be adhered to. This is a combined synopsis/solicitation, Request for Quotation (RFQ), for commercial items prepared in accordance with FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. The applicable North American Industry Classification System (NAICS) Code and size standards in millions of dollars are 561621 and $22.0, respectively. FAR 52.212-2, Evaluation - Commercial Items is applicable to this procurement. This requirement will be awarded to a responsible contractor whose proposal represents the best value to the Government, considering price and technical factors, with technical and past performance factors more important than price. The evaluation of offers received in response to the solicitation will use a tiered or cascading order of precedence. Tiered evaluation of offers, also known as cascading evaluation of offers, is a procedure used in negotiated acquisitions when market research is inconclusive for justifying limiting competition to small business concerns or sub-categories of small business concerns. The contracting officer (1) Solicits offers from both small and other than small business concerns that will be evaluated in the following tier order: (a) service-disabled veteran-owned small business (SDVOSB); (b) veteran-owned small business (VOSB); (c) all other small business concerns; (d) other than small business. STATEMENT OF WORK INTRODUCTION AND SCOPE OF WORK This service contract is to provide all labor, tools, material, and special equipment necessary to perform a complete 5 Year Standpipe inspection and FDC Check Valve testing on fire sprinkler system and provide documentation certifying compliance with current National Fire Protection Association (NFPA) -25 (latest edition) and The Joint Commission (TJC) - latest edition or other applicable current recommendations. In the event such systems and equipment do not meet requirements of the previous reference documents or other applicable documents, contractor will issue certifying documentation subject to listed recommendations (provide a summary sheet detailing discrepancies and recommendations to resolve discrepancies) to bring system and equipment into compliance with aforementioned standards. Work will be performed at the Department of Veterans Affairs (VA) Medical Center, 7305 N. Military Trail, West Palm Beach, FL. REQUIREMENTS Prior to any worker for the contractor begins work, the contractor prepares a safety plan and ensures workers understand and comply with the safety measures while working at VA Medical Center fire sprinkler system and equipment. Perform a 5yr obstruction inspection on 17 Standpipe Risers and flush as required by NFPA 25. Flushing procedure to start at FDC or other riser connection. Inspection consists of looking inside the piping for visible obstructions, attaching 2 ½ fire hoses to the end fitted with a bag to catch any debris, running water through the system with all control valves open. If any debris is found, then contractor will follow piping back to next bend, separate for further investigation until source of debris is found. If no debris is found the system is determined to be Clear . In the event the contractor finds leaking piping or piping breakage during inspection, a fixed price estimate will be provided for the cost of the repair. Affix a tag to the rooftop manifold after testing is completed. Perform a 5yr obstruction inspection on the Fire Department Connection Check Valves as required by NFPA 25. Perform 5yr hydrostatic testing of 17 FDCs piping from the FDC to the check valve as required by NFPA 25. Test pressure will be 150 psi for (2) two hours. Provide a TJC Complaint Hydrostatic test report. Perform a full flow fire pump test on (1) Fire Pump. Provide a fire pump curve and TJC Complaint report. All work is scheduled to be performed during normal hours of VA operations. The fire sprinkler system will not be left offline longer than 8hrs in a 24hr period. The work will take approximately 4 days to All written reports will be clear and legible and reflect how the measurements were made, where they were made (locations) and compared with specified standards for each test. Test results will be in an easy to read format that is recognized by the Joint Commission. DELIVERY Performance of services shall be rendered as identified above. Scheduling of services shall be coordinated with the Plumbing Shop Supervisor, telephone 561-422-6655. Contractor shall report to the Plumbing Shop Supervisor, commencing work. PROGRESS AND COMPLIANCE Contractor shall perform services on this order only to the extent indicated on this agreement. No additional services/repairs will be made without the approval of the Plumbing Shop Supervisor and the Contracting Officer. Contractor shall provide service tickets, at the time that services are rendered, for all services performed. These services tickets shall be signed by a Plumbing Shop employee in order to confirm services were rendered. All payments shall be made in arrears. Upon receipt of Contractor s invoices, the Plumbing Shop Supervisor will certify invoices through referenced service tickets, assuring that all services were performed in accordance with contract terms. Additional requirements: All work shall be in accordance with NFPA, TJC, VA standards, and other related codes. All work shall be completed within 90 days from contract award. All shutdowns shall be coordinated with COR in advance Workmen are subject to all the rules of the Medical center. Contractor tools and equipment shall be continuously under direct line of sight supervision of the contractor. NO tools / equipment can be left at the individual work sites over-night must put away each day. Contractor shall clean-up work sites DAILY. NO debris can be left at work sites. A site visit is highly recommended to allow prospective contractors to verify all existing conditions and include all such implications in the proposal. The Contracting Officer is the only one authorized to schedule any site visit.
