This announcement constitutes the only solicitation; offers are being requested and a written solicitation will not be issued. The solicitation number 36C24821Q1394 is issued as a Request for Quotation (RFQ) in conjunction with FAR -- Part 13 Simplified Acquisition Procedures and constitutes the only solicitation. The Government intends to award a single Firm-Fixed Price (FFP) award contract. The provisions and clauses in this document are in effect through the Federal Acquisition Circular 2021-04, January 19, 2021. This solicitation intends to award a contract through full and open competition after exclusions under a SB set aside to a business associated with NAICS Code 334519 with a size standard of 500 employees. The Department of Veterans Affairs (VA) seeks to make a procurement for Radiation Dosimetry Badges for the West Palm Beach VA Medical Center, Palm Beach, FL. See Performance Work Statement Below and Pricing List Below: Performance Work Statement 1.0. Introduction. 1.1. The U.S. Nuclear Regulatory Commission (NRC) regulations and the permit to use byproduct material under the Masters Materials License (MML) issued to the Department of Veterans Affairs issued to the West Palm Beach Veterans Affairs Medical Center, require that each permittee monitor exposures to radiation and radioactive material at levels sufficient to demonstrate compliance with the occupational dose limits set by NRC. At a minimum, each permittee shall monitor occupational exposure to radiation from licensed and unlicensed radiations sources under the control of the permittee and shall supply and require the use of individual monitoring devices. It should be noted that a dosimetry program is highly regulated by the Nuclear Regulatory Commission (NRC). Maintaining a dosimetry program requires using a reliable dosimetry processor. Failure to maintain an appropriate dosimetry program and provide individual monitoring devices or equipment could result in severe penalties, lawsuits, fines, and/or the revocation of our permit to use byproduct material under the MML issued to the Department of Veterans Affairs by the NRC. Loss of our permit would result in the termination of all Nuclear Medicine Clinic Operations, all other clinical and therapeutic uses of radioactive materials, and the research use of radioactive materials if applicable. Penalties and fines could also be levied against your medical center along with the fees and expenses resulting from decommission of a NRC Byproduct Materials License. 2.0. Description / Scope / Objective. 2.1. The purpose for this contract is to have a dosimetry processor supply, process, evaluate, package, and ship dosimeters, as well as maintain records for dosimeter exposures and supply necessary reports of exposures in an efficient manner which meet all applicable regulatory requirements. The dosimetry processor must be able to demonstrate and provide the licensing and specific requirements listed below prior to the award of the contract. 2.1.1 The term dosimetry processor means an individual or an organization that processes and evaluates individual monitoring equipment in order to determine the radiation dose delivered to the equipment. 2.1.2 The term individual monitoring device (radiation monitoring, radiation protection monitoring) means devices designed to be worn by a single individual for the assessment the measurement of radiation levels, concentrations, surface areas concentrations or quantities of radioactive material and the use of the results of these measurements to evaluate potential exposures and doses. 2.1.3 The term occupational dose means the dose received by an individual in the course of employment in which the individual s assigned duties involve exposure to radiation or to radioactive material from licensed and unlicensed sources of radiation, whether in the possession of the licensee (permitee) or other person. 3.0. Applicable Documents. (provide a list of documents, policies, etc. which affect this requirement) 3.1. Performance Requirements. 4.1. Licensing Requirements: 4.1.1. The dosimetry processor must hold a current personnel dosimetry accreditation from the National Voluntary Laboratory Accreditation Program (NVLAP) of the National Institute of Standards and Technology (NIST) in all categories. 4.1.2. The dosimetry processor must be approved in this accreditation process for the type of radiation or radiations included in the NVLAP program that most closely approximates the type of radiation or radiations for which the individual wearing the dosimeter is monitored. 4.1.3. The dosimetry processor must be able to calculate, provide and store the deep-dose equivalent to the whole body, lens of the eye dose equivalent, shallow-dose equivalent to the skin, and the shallow-dose equivalent to the extremities. 4.1.4. The dosimetry processor must be able to calculate, provide and store the following information: 4.1.4.1. Committed Effective Dose Equivalent (CEDE) organ 4.1.4.2. Committed Dose Equivalent (CDE) total 4.1.4.3. Total Organ Dose Equivalent (TODE) 4.1.4.4. Total Effective Dose Equivalent (TEDE) 4.1.5. The dosimetry processor must be able to store the following information: 4.1.5.1. Committed Dose Equivalent (CDE) organ 4.1.5.2. Committed Effective Dose Equivalent (CEDE) organ 4.1.5.3. Intake quantity and Radionuclide 4.1.5.4. Solubility Class 4.1.5.5. Description of Internal Deposition 4.1.6. The dosimetry processor must be able to provide evidence of the following: 4.1.6.1. Maintain an in-house quality assurance program. 4.1.6.2. Minimum five years experience as a dosimetry processor. 4.1.6.3. Satisfactory performance record with NVLAP and NIST in all eight categories. 4.2. Specific Requirements: 4.2.1. The dosimetry processor must provide the following: 4.2.1.1 Aluminum oxide, optically stimulated luminescence (OSL), whole-body dosimeters that can assess x-ray, gamma and beta exposure. 4.2.1.2 Whole-body dosimeters must be able to detect a dose of 1 mrem from photons with energies ranging from 5 keV to in excess of 40 MeV. 4.2.1.3 Whole-body dosimeters must be able to detect a dose of 10 mrem from beta particles with energies ranging from 150 keV to in excess of 10 MeV. 4.2.1.4 Whole-body dosimeters must not be affected by heat, moisture and pressure. 4.2.1.5 Lithium fluoride thermoluminescent dosimeters (TLD) for assessing extremity exposure. 4.2.1.6 Aluminum oxide dot or small sized dosimeters with onsite reader device for rapid radiation assessment. 4.2.1.7 Neutron detection dosimeters that can assess fast, intermediate and thermal neutron exposure. Neutron dosimeter shall have the capacity to be integrated into aluminum oxide whole-body dosimeter. 4.2.1.8 Aluminum oxide whole-body dosimeters with emergency response packaging. 4.2.1.9 Master controls (dosimeters used for assessing background, transit and storage) for all types of dosimeters. 4.2.1.10 Aluminum oxide or TLD environmental dosimeters. 4.2.1.11 Dosimeters will have permanent waterproof ID labels that cannot be smeared and/or removed with barcode capability. 4.2.1.12 Whole-body dosimeters must not be sensitive to heat so as not to yield false positive results when exposed to environmentally high temperatures. 4.2.1.13 All radiation dosimetry services must meet current 10 CFR 20 reporting and recording requirements. 4.2.1.14 Reports of radiation exposures must be made on NRC Form 4 and NRC Form 5 or equivalent. Reports must also be supplied electronically upon request in a format which meets the NRC "REMIT" format. 4.2.1.15 On-line support for on-line exposure reports, shipment tracking and account maintenance transactions (i.e. dosimeter add/delete/reactivate administrative capabilities, spare badge assignments, addition of previous exposure history to current records). 4.2.1.16 On-line support shall provide unlimited access to help-line . 4.2.1.17 Absent badge reports for each wear period can be supplied within 24 hours within receipt of dosimeters. 4.2.1.18 Twenty-four-hour emergency dosimeter processing service upon request. 4.2.1.19 Exposure reports shall be supplied within 15 working days from receipt of dosimeters. 4.2.1.20 A separate fetal dosimetry monitoring program which includes cumulative totals on exposures from year to year. 4.2.1.21 As Low As Reasonably Achieveable (ALARA) notification and memos for high exposures for any processed dosimeter exceeding specified ALARA trigger limits. 4.2.1.22 Termination report summaries of personnel radiation exposures shall be supplied within 30 days of terminating an employee badge service. 4.2.1.23 ALARA report summaries will be provided on a quarterly basis with an additional annual ALARA review summary for all monitored employees for all accounts. 4.2.1.24 Dosimeters shall be supplied seven days before the beginning of any designated wear period to allow time for distribution once received. 4.2.1.25 A representative of the company will provide site visits and software training once each year at a minimum. 4.2.1.26 Monthly report listing individuals who have an activity in other accounts during a reporting year. Reports shall sum each individual s exposures across all facilities for comparison with regulatory limits. 4.2.1.27 Reports of dosimeters that are unreturned during a wear period. 5.0. Performance Location and Hours. 5.1. Primary place of performance is Contractor location. 6.0. Deliverables. N/A 7.0. Key Personnel Replacement and Substitution. N/A 8.0. Contractor Personnel and Management. 8.1. Contractor Personnel. The Contractor shall be responsible for employing technically qualified personnel to perform the work specified in this PWS. The Contractor shall maintain the personnel, organization and administrative control necessary to ensure that the work delivered meets the contract specifications and requirements. 8.3. Government Badges. The COR will coordinate the issuance of required access badges to Contractor personnel. Contract personnel shall wear laminated identification badges provided by the Government at all times when performing. Badges shall be worn on the outer garment, attached to the outer shirt or jacket pocket by a button or clip or worn around the neck secured by an appropriate identification card lanyard. 8.3.1. Government issued identification badge remains the property of the Government and shall be returned to the issuing office or other designated Government personnel upon transfer of Contractor employees from the contract. 8.5. All Contractor personnel attending meetings, answering Government telephones, and working in other situations where their Contractor status is not obvious to third parties are required to identify themselves as such to avoid creating an impression in the minds of members of the public or Congress that they are Government officials. They must also ensure that all documents or reports produced by Contractor personnel are suitably marked as Contractor products or that Contractor participation is appropriately disclosed. 8.6. Safety/Security Requirements. The Contractor shall comply with all applicable federal, state and local laws and ensure performance is secure while protecting material, equipment, and supplies from damage and loss. Government security personnel shall have the express right to inspect for security violations at any time during the term of the contract. 8.7. All information generated in performance of this contract becomes property of the Government and must be surrendered upon request and/or completion of the contract. The Contractor must protect against the destruction and/or misplacement of all information produced under this contract, unless directed under local policy to destroy. This includes both hard and soft copies of data. 8.8. The Contractor shall not release any information (including photographs, files, public announcements, statements, denials, or confirmations) on any part of the subject matter of this contract or any phase of any program hereunder without the prior written approval of the COR. 8.9. Standards of conduct 8.9.1. Conduct of Personnel. Contractor personnel shall conduct themselves in a professional manner (i.e. timeliness, communication; spoken and written etc.). The Contracting Officer may require the Contractor to remove from the job site any employee working under this contract for reasons of suspected misconduct, a suspected security breach, or suspected to be under the influence of alcohol, drugs, or any other incapacitating agent. Contractor employees shall be subject to dismissal from the premises upon determination by the Contracting Officer that such action is necessary in the interests of the Government. The hospital director or designated representative has the authority to bar individuals from the Government facility. The removal from the job site or dismissal from the premises shall not relieve the Contractor of the requirement to provide sufficient personnel to perform the services as required by this performance work statement. The Government shall not reimburse the Contractor for travel and other expenses associated with the removal of personnel. 8.9.2. Contractor personnel are not authorized to carry or possess personal weapons to include, but not limited to, firearms and knives with a blade length in excess of three inches, while assigned under this contract. 8.9.3. Working Attire and Appearance. Contractor personnel shall present a professional appearance commensurate with standards delineated for Government civilian personnel acting in similar capacities. 8.9.4 The Government reserves the right to review Contractor personnel qualifications. Upon request, resumes shall be provided to the Contracting Officer prior to clearance approval, for review and acceptance by the COR. The intent is verification purpose and not for the Government to become the hiring authority. 9. TRAVEL N/A 10. NON-PERSONAL SERVICE STATEMENT 10.1. Contractor employees performing services under this order will be controlled, directed, and supervised at all times by management personnel of the contractor. Contractor management will ensure that employees properly comply with the performance work standards outlined in the PWS. Contractor employees will perform their duties independent of, and without the supervision of, any Government official. The tasks, duties, and responsibilities set forth in the contract may not be interpreted or implemented in any manner that results in any contractor employee creating or modifying Federal policy, obligating the appropriated funds of the United States Government, overseeing the work of Federal employees, providing direct personal services to any Federal employee, or otherwise violating the prohibitions set forth in Parts 7.5 and 37.1 of the Federal Acquisition Regulation (FAR). The Government will control access to the facility and will perform the inspection and acceptance of the completed work. 11. PERSONNEL QUALIFICATIONS 11.1. The Contractor shall be responsible for employing technically qualified personnel to perform the work specified in this PWS. The Contractor shall maintain the personnel, organization, and administrative control necessary to ensure that the work delivered meets the contract specifications and requirements. The work history of each contractor employee must contain experience directly related to the task and functions he/she is intended to perform under this contract. The Government reserves the right, during the life of the resulting contract, to request work histories on any contractor employee for the purposes of verifying compliance with the above requirements; additionally, the Government reserves the right to review resumes of contractor personnel proposed to be assigned. Personnel assigned to, or utilized by, the Contractor in performance of work shall be fully capable of performing the requirements contained in the PWS in an efficient, reliable, and professional manner. The normal manner of dress is business casual. 11.2. Specific Personnel Qualifications Requirements Personnel assigned to or utilized by the contractor in the performance of this contract shall, as a minimum: 11.2.1. Be a United States Citizen 12. PERFORMANCE STANDARDS AND QUALITY MEASUREMENT 12.1. Performance standards define desired services. The Government performs surveillance to determine if the Contractor exceeds, meets, or does not meet these standards. The Government shall use these standards to determine Contractor performance and shall compare Contractor performance to the Acceptable Quality Level (AQL). The Quality Assurance Surveillance Plan (QASP) method of surveillance will be by random inspection. 12.2 The QASP and its performance objectives are as follows: (SAMPLE) PWS Para.
