Page 8 of 8 COMBINED SYNOPSIS/SOLICITATION The West Palm Beach VA Medical Center located at 7305 N Military Trail, West Palm Beach, FL 33410 has a requirement to provide all labor, tools, material, and equipment necessary for a contractor remove existing Air Curtains and install new larger capacity Air Curtains at both sides of the employee entrance on the east side basement of the Medical Center. Contractor to install new air curtains at the Main Entrance of the Medical Center, east side. Specific scope of the project is listed below. Work will be performed at West Palm Beach, Veterans Affairs Medical Center, 7305 N. Military Trail, West Palm Beach, FL. The attached Statement of Work (SOW) shall be adhered to. This is a combined synopsis/solicitation, Request for Quotation (RFQ), for commercial items prepared in accordance with FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. The applicable North American Industry Classification System (NAICS) Code and size standards in millions of dollars are 238210 and $16.5Million, respectively. FAR 52.212-2, Evaluation - Commercial Items is applicable to this procurement. This requirement will be awarded to a responsible contractor whose proposal represents the best value to the Government, considering price and technical factors, with technical and past performance factors more important than price. The Government will select the quote that represents the best benefit to the Government at a price that can be determined reasonable. Once the Government determines there is/are a contractor(s) that can provide a product/service that meet(s) the requirements of this request for quote, the Government reserves the right to communicate with only those contractors quoting the best-suited product/service to address any remaining issues. Evaluation factors will be considered in descending order of importance: Technical Capability: Provide technical performance plan and proof of applicable certifications, license, and insurance. Past Performance: Prospective contractors may provide a list of five business references for which the contractor performed the same or similar scope as required in the performance work statement. The reference information shall include: name of business; name and title of point of contact; brief description of services performed; and date services performed. Contractor s Proposed Rate for: Remove existing two (2) air curtains in the basement east employee entrance and install two new industrial air curtains. Add air curtain in east entrance of the Main Hospital. Air curtain to be 72 wide, minimum 2700 CFM. Work will include all new necessary electrical work to power the new air curtains as well as tie into existing automatic doors. All work under this contract shall be fully covered by a one-year parts and labor warranty. REQUIREMENTS The contractor shall furnish all Supervision, site safety and oversite for the following: Remove existing two (2) air curtains in the basement east employee entrance and install two new industrial air curtains. Air curtains to be 72 wide each, minimum 2700 CFM. Work will include necessary electrical work to power the new air curtains. Contractor may use existing conduit run to the air curtains, however, will be responsible for properly sizing breaker and wire. Add air curtain in east entrance of the Main Hospital. Air curtain to be 72 wide, minimum 2700 CFM. Work will include all new necessary electrical work to power the new air curtains as well as tie into existing automatic doors. Contractor may use existing conduit if available, however will be responsible for properly sizing breaker, conduit and wire. Contractor shall completely prepare the site for this project, including implementation of dust control and other construction safety measures, and furnish labor, materials and equipment necessary to complete all work which includes but not limited to verification of proper operation of existing equipment. Contractor will perform Operational Verification in the presence of the Contracting Officers Representative. Contractor will be required to turn over operational and maintenance manuals prior to final acceptance and payment. DELIVERY Performance of services shall be rendered as identified above. Scheduling of services shall be coordinated with the Contracting Officer Representative (Mr. Jon Roberts 561-422-1482). A minimum of 7 day notice is required prior to commencement of any work. Contractor shall report to the FMS Engineering Department daily prior to commencing work. PROGRESS AND COMPLIANCE Contractor shall perform work on this statement of work only to the extent indicated on this agreement. No additional services/repairs will be made without the written approval of the Contracting Officer. All product submittals must be submitted to the COR for approval. Contractor shall provide daily report (s), for all services performed. These daily report (s) shall be provided to the Contracting Officer Representative (COR) in a timely manner. All payments shall be made in arrears. Upon receipt of Contractor s invoices, the COR will certify invoices through referenced service tickets, assuring that all services were performed in accordance with contract terms. ADDITIONAL REQUIREMENTS: All work shall be in accordance with Occupational Safety and Health Administration (OSHA), VA standards, and other related codes. All shutdowns (electrical) shall be coordinated with COR. Workmen are subject to all the rules of the Medical center and expected to use personal protective equipment when necessary. Contractor tools and equipment shall be continuously under direct line of sight supervision of the contractor. NO tools / equipment can be left at the individual work sites overnight. Contractor shall clean-up work sites daily. No debris can be left at work sites. Hot Work, Infection Control and Interim Life Safety permits shall be issued as required by the VA s COR. Before starting work the General Contractor shall give one week s notice to the Contracting Officer so that security arrangements can be provided for the employees. General Contractor s employees shall not enter the project site without appropriate badge. They may also be subject to inspection of their personal effects when entering or leaving the project site. WARRANTY: All work under this contract shall be fully covered by a one year parts and labor warranty. **** It is recommended that the Contractor visit the work site (s) and verify all existing conditions, measurement, equipment, and include all such implications in the proposal.
