This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued. Solicitation (36C24821Q0022) is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2020-05. This solicitation is being issued as a tiered evaluation for small business concerns in accordance with 38 U.S.C. 8127 (i). If no offers are submitted by other small business concerns, or if none of the offers result in award at a fair and reasonable price that offers best value to the government, the solicitation will be cancelled and resolicited as an unrestricted procurement. The associated North American Industrial Classification System (NAICS) code for this procurement is 621111, Offices of Physicians (except Mental Health Specialists), with a small business size standard of $12 million. The Product Service Code (PSC) is Q999, Medical Other Medical Services. NAICS code 621111, shall be represented in contractor s System for Award Management profile registration prior to award. The Service Area Office (SAO) East, Network Contracting Office (NCO 8), Tampa, Florida is seeking to acquire on-site Complete Intraoperative Neuromonitoring Service (IONM) to include on-call / call back coverage in support of the Bruce W. Carter Veterans Affairs Medical Center ( hereinafter to referred to as BWCVAMC), Operative Care Department located at 1201 Northwest 16th Street, Miami, FL 33125 from a contractor that can meet/exceed the government s objectives for the requirement. Required services will be performed at the BWCVAMC. The contractor will be required to provide the equipment, labor, materials, accessories, and supplies necessary for Complete IONM services in support of seven (7) surgical operating rooms. The Complete IONM services are required as scheduled or on an as needed/case-by-case which enables the BWCVAMC to offer IONM options to patients under direct medical care within its operating room. The umbrella of IONM services includes, but is not limited to, complex surgical procedures such as: Spine surgery, Neurosurgery, Vascular Surgery, Otolaryngology, Cardiac Surgery, Thoracic Surgery, Orthopedics, and if necessary, Interventional Neuroradiology. The contractor shall be able to provide on-site Complete IONM services during scheduled timeframes, and during emergencies 24 hours a day, seven (7) days a week, & 365 day a year. All work shall be completed in accordance with the Performance Work Statement (PWS) entitled. The PWS contained within this solicitation is a non-negotiable document. Quotes shall not include edits, revisions, additions, deletions, alterations, or track change comments to the PWS.
