The purpose of this email is to gain knowledge of potential qualified sources and their size classification relative to the North American Industry Classification Code (NAICS) 325416, (size standard of 1,250 employees). Responses to this notice will be used by the Government to make appropriate acquisition decisions. Responses to this notice is not considered adequate responses to the solicitation announcement. The Government will not pay nor reimburse any costs of preparing this response. The Department of Veterans, Network Contracting Office 8, Orlando VA Healthcare System has a requirement for the Caribbean VA Healthcare System, 10 Casia Street, San Juan, PR 00921. NCO 8 has the need for a requirement contract with a base plus four-year option for Cost Per Test: I-Stat s Point of Care. You must be an authorized distributor of the products for the items of your submittal. You will be required to provide proof of designation or official letter from the manufacturer. Can you provide the following items? Provide one instrument for Creatinine Cartridge, then provide reagents and consumables to perform a panel of test to include: EG6+ Reagents for Blood gases that include Ph, PCO2, PO2, Sodium, Potassium, TCO2 Hematocrit, Hemoglobin CHEM8 reagent that include: Sodium, Potassium, Chloride, Glucose, Bun, Creatinine, ionized Calcium, Total CO2, Hematocrit, Hemoglobin, Anion Gap, Glucose reagent ACT-Kaolin Creatinine Product Quarter Volume EG6+ (03P77-25) 2,250 Glucose (03P83-25) 625 ACT-K (03P87-25) 350 CHEM8 (09P31-25) 1,500 Creatinine (03P84-25) 2,400 The performance period for this contract will be one year plus four option years.
