Description: This is a combined synopsis/solicitation for commercial products or commercial services prepared in accordance with the format in subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. 1) Solicitation 36C24723Q0343 is issued as a request for quotation (RFQ). 2) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2023-01, Effective 12/30/2022. 3) This solicitation is being posted as a set-aside for service-disabled veteran owned small businesses (SDVOSBs). The associated NAICS code is 524292 and small business size standard is $42.5 Million. 4) The government will award a single firm fixed-priced purchase order for Emergent Prescription Fill Services to VA beneficiaries of the Ralph H. Johnson VA Medical Center Community Based Outpatient Clinics (CBOCS). The period of performance will be for one base year with an additional four one-year option periods. 5) A full description of this requirement, including (but not limited to) the statement of work, contractor responsibilities, terms and conditions, provisions, clauses , addenda, instructions, and qualifications are included in the attached solicitation. It is the offerors responsibility to review the attached solicitation (36C24723Q0343) for a full description of this requirement before submitting a quote.
