The contractor shall provide autopsy services including covid-19 related for Atlanta and Dublin Veterans Health Care Systems. The North American Industry Classification System (NAICS) code applicable to this acquisition is 621511 Medical Laboratories Small Business Size Standard, $35.0 M. The product Service Code is Q515 Pathology. Period of performance is 12 months from the date of the Contracting Officer signature, plus 2 12 month option periods. The awardee will be responsible for submitting certified proof of employment for any submitted candidate to validate the candidate is employed by the awardee. This requirement is due 60 days after the date of award and 30 days after the date each substitute candidate is submitted. Failure to provide this information within the specified timeline may result in termination for non-compliance with the 50 percent labor cost requirement stated in VAAR Clause 852.219-10. Personnel assigned by the Contractor to perform the services covered by this contract shall be licensed in a State, Territory, or Commonwealth of the United States or the District of Columbia. The qualifications of such personnel shall also be reviewed by Atlanta and or Dublin Professional Standards Board and approved by the Atlanta or Dublin Director prior to being authorized to provide patient care services. The licensee shall have an active, current, full and unrestricted license to practice his/her profession. License - Contract physician(s) assigned by the Contractor to perform the services covered by this contract shall have a current license to practice medicine in a State, Territory, Commonwealth of the United States, or in the District of Columbia. All licenses held by the personnel working on this contract shall be full and unrestricted licenses. Contract physician(s) who have current, full and unrestricted licenses in one or more states, but who have, or ever had, a license restricted, suspended, revoked, voluntarily revoked, voluntarily surrendered pending action or denied upon application will not be considered for the purposes of this contract. All contractor personnel must have general Liability and Malpractice Insurance. The VA may request the personnel records of any employee providing services under contract for competency review. The VA reserves the right to request copies of current certifications and/or licenses at any time during the duration of this contract. This requirement will be issued as a Firm Fixed Priced Type contract 100% set-aside for woman owned small Business (WOSB) concerns. The solicitation will be issued on or about November 10, 2022. The estimated closing date is November 15, 2022. The solicitation will not be mailed out. All interested sources must go to the SAM.gov / Opportunities and obtain an electronic copy of the solicitation. Offerors are responsible to ensure their proposal package is received by the exact time and date at the specified location for receipt of proposals. In addition, offerors are responsible for continuing to monitor the websites for any/all future Amendments issued, if any. Offerors shall sign copies of any amendment issued and submit with their package. Offerors need to be registered in the System for Award Management (SAM) database at https://sam.gov/SAM/ prior to submission of proposal. All questions shall be in writing to the Contracting Officer.
