To that end, the support contractor(s) may require access to contractor's offices where the contractor's business records or other proprietary data are retained and to review such business records regarding the contractor's compliance with this requirement. (End of Clause) SUPPLEMENTAL INSURANCE REQUIREMENTS In accordance with FAR 28.307-2 and FAR 52.228-5, the following minimum coverage shall apply to this contract: (a) Workers' compensation and employer s liability: Contractors are required to comply with applicable Federal and State workers' compensation and occupational disease statutes. If occupational diseases are not compensable under those statutes, they shall be covered under the employer's liability section of the insurance policy, except when contract operations are so commingled with a Contractor's commercial operations that it would not be practical to require this coverage. Employer's liability coverage of at least $100,000 is required, except in States with exclusive or monopolistic funds that do not permit workers' compensation to be written by private carriers. (b) General Liability: $500,000 per occurrences. (c) Automobile liability: $200,000 per person; $500,000 per occurrence and $20,000 property damage. (d) The successful bidder must present to the Contracting Officer, prior to award, evidence of general liability insurance without any exclusionary clauses for asbestos that would void the general liability coverage. Contractor will provide at contractor s expense all tools, materials, supplies, equipment and labor to perform the work. PREVENTIVE MAINTENCE & REPAIR: The contractor shall provide all supervision, labor, equipment and supplies necessary to perform a minimum of 40 hours preventive maintenance (PM) per month at the Birmingham VA Medical Center (521). Preventive maintenance to be scheduled at least three (3) days in advance with the Contracting Officer s Representative (COR). Repair time is not to be included as preventive maintenance time. Preventive maintenance shall be performed in accordance with manufacturer s instructions and comply with the most current OSHA, ASME 17.1 8.6 guides and all applicable codes for the Preventive Maintenance, Repair and safety of elevators and dumbwaiters and will include the following: a. Visual inspections b. Calibration checks c. Cleaning and lubrication d. Electrical checks e. Performance checks f. 5 year load test (option year 3) g. Annual elevator inspection, Via 3rd party. Performance period: Base Year 1 Oct 2022 30 Sep 2023 Elevators to be serviced: 1, 2, 3, 4, 5, 6, 7, 8, 9, 10, 11, 12, 13, 14, 15, 16, 17, 18, T1, T2, T3 Option Year 1: 1 Oct 2023 30 Sep 2024 Elevators to be serviced: 1, 2, 3, 4, 5, 6, 7, 8, 9, 10 ,11, 12, 13, 14, 15, 16, 17, 18, T1, T2, T3 Option Year 2: 1 Oct 2024 30 Sep 2025 Elevators to be serviced: 1, 2, 3, 4, 5, 6, 7, 8, 9, 10, 11, 12, 13, 14, 15, 16, 17, 18, T1, T2, T3 Option Year 3: 1 Oct 2025 30 Sep 2026 Elevators to be serviced: 1, 2, 3, 4, 5, 6, 7, 8, 9, 10, 11, 12, 13, 14, 15, 16, 17, 18, T1, T2, T3 Option Year 4: 1 Oct 2026 30 Sep 2027 Elevators to be serviced: 1, 2, 3, 4, 5, 6, 7, 8, 9, 10, 11, 12, 13, 14, 15, 16, 17, 18, T1, T2, T3 Elevators and Dumbwaiters located at DVAMC Birmingham: REPORTING: For repairs or services that will be performed during normal working hours, the Contractor s repairman must report to the Mechanical Shop Engineering Section, room BC104, (telephone extension 335600), to report/sign-in and to obtain an Identification Badge which shall be worn at all times while repairman is on station. After all work is completed, the Contractor s repairman must again report in person to the Mechanical Shop Engineering Section, room BC104, to submit in writing, a complete report of services rendered and to return the issued ID Badge. At a minimum, the report must contain a detailed description of all services or repairs performed for each item of equipment and must also include a listing of replacement parts, when applicable. The service report must be signed by the Mechanical Shop Supervisor or his designee before leaving the facility. For repairs or services required after normal working hours the Contractor s repairman is required to report to the Mechanical Shop Engineering Section, room BC104 to the Engineer on Duty (Unit 5) to obtain an ID Badge. Repairman is still required to complete a report of service rendered and leave it with the Engineer on Duty (Unit 5) in the Mechanical Shop Engineering Section, room BC 104. Repairman will not leave the report of service at the equipment s location. Contractor s repairman will leave ID badge with the Engineer on Duty (Unit 5) service personnel upon leaving the facility. Both the check-in and check-out procedures defined above are mandatory and will be strictly enforced. PERFORMANCE: The Contractor must respond to service calls within two (2) hours of notification and be on-station within (8) hours. All work must be performed by competent personnel who are experienced and qualified to work on the specific equipment. All work performed shall be accomplished in accordance with the manufacturer s instructions including, but not limited to, adjustments, calibrations, cleaning, lubrication, testing, disassembly, check-out, replacement of worn or defective parts, etc., required to keep the equipment in operating condition. Contractor s maintenance of the equipment must satisfy all requirements as set forth by NFPA, OSHA, ASME and JCAHO. HOURS OF WORK: This contract shall include all necessary service calls seven (7) days per week, between the hours of 7:00a.m. and 5:00p.m. Central Standard Time, Federal Holidays included.
