SYNTHES 7.3MM SCREW SET (IMPLANTS) Statement of Work (SOW) Page 2 of 3 GENERAL INFORMATION Purpose: Purchase Synthes 7.3mm Crew Set. Scope of Work: The contractor shall provide all resources necessary to accomplish the deliverables described in this statement of work (SOW), except as may otherwise be specified. Contractor shall procure and deliver one (1) Synthes 7.3mm Screw Set as identified in this and the salient characteristics document. Background: Sterile Processing Service (SPS) Service Line has been tasked to provide 1 Synthes 7.3mm Screw Set. Performance Period: This is a one-time purchase; delivery should be completed within sixty (60) days from date of award. Preferably no partial orders. Type of Contract: Firm-Fixed-Price. Place of Performance: Delivery will be made to Charlie Norwood VA Medical Center, 1 Freedom Way, Augusta GA 30904. Hours of Operation: Warehouse deliveries are accepted between 8am and 4:00pm, Monday through Friday, excluding holidays. Hospital normal hours of operation are between 8am and 4:30pm, Monday through Friday, excluding holidays. Holidays falling on a Sunday will be observed the following Monday. CHANGES TO STATEMENT OF WORK Any changes to this SOW shall be authorized and approved only through written correspondence from the CO. A copy of each change will be kept in a project folder along with all other products of the project. Costs incurred by the contractor through the actions of parties other than the CO shall be borne by the contractor. D. INVOICES 1. Payment will be made upon receipt of a properly prepared detailed invoice, prepared by the Contractor, validated by the Contracting Officer s Representative (COR), and submitted electronically through OB-10 (https://portal.tungsten-network.com/). 2. A properly prepared invoice will contain: a) Invoice Number and Date b) Contractor s Name and Address b) Accurate Purchase Order Number c) Supply or Service provided d) Total Amount Due E. SECURITY PER REVIEW OF VA HANDBOOK 6500.6, CONTRACT SECURITY, APPENDIX A- INFORMATION SECURITY AND PRIVACY CHECKLIST: The C&A requirements do not apply and a Security Accreditation Package is not required. Acquisition of this service does not involve the storage, generating, transmitting, or exchanging of VA sensitive information to the vendor. G. CONFIDENTIALITY AND NONDISCLOSURE It is agreed that: The preliminary and final deliverables and all associated working papers, application source code, and other material deemed relevant by the VA which have been generated by the contractor in the performance of this task order are the exclusive property of the U.S. Government and shall be submitted to the CO at the conclusion of the task order. The CO will be the sole authorized official to release verbally or in writing, any data, the draft deliverables, the final deliverables, or any other written or printed materials pertaining to this task order. No information shall be released by the contractor. Any request for information relating to this task order presented to the contractor shall be submitted to the CO for response. Press releases, marketing material or any other printed or electronic documentation related to this project, shall not be publicized without the written approval of the CO.
