The Birmingham VA Medical Center anticipates a requirement to provide Transitional House services to beneficiaries authorized by the Department of Veteran Affairs. This contract will have a performance period of a base year plus four (4) option years and will be Firm Fixed Priced (FFP). The performance will occur at the Contractor’s location and Government furnished property will not be provided. Health Care for Homeless Veterans (HCHV) provides case management services to homeless Veterans and facilitates their access to a broad range of medical, substance abuse, mental health, rehabilitative, and employment services. HCHV’s goal is to engage homeless Veterans in supportive services and place them in a safe and secure environment that supports their rehabilitation goals. Care will include, but not be limited to: Mental Health Substance Abuse Treatment Employment Assistance
