DEPARTMENT OF VETERANS AFFAIRS NCO7 Contracting Office 3701 Loop Road East Tuscaloosa, AL 35404 March 10, 2021 Prospective Offerors Subject: RFQ: 36C24721Q0418 Preventative Maintenance for Ceiling Lifts Dear Prospective Offerors: The VA Medical Center invites your firm to submit a quote for the subject requirement as indicated in the Price/Cost Schedule of the solicitation. The Government intends to make an award of a firm-fixed price contract for this procurement. Please submit a quote in accordance with the following instructions: Complete blocks 12, 17a, 30(a) (b) and (c) of the attached SF1449. In doing so, the offeror accedes to the contract terms and conditions as written in the Solicitation. The Solicitation constitutes the model purchase contract. Complete Contractor information on page 3 of the solicitation Complete PRICE/COST SCHEDULE starting on page 4 of the solicitation. Submit quotes on company letterhead. Your reply to this request for quote must set forth full, accurate, and complete information as required by this solicitation. Your quote shall be signed by an official authorized to bind the firm and be submitted via email on or before 8:00 AM. CST on Friday, March 12, 2021.
