Page 2 of 2 Pre-Solicitation Notice The Department of Veterans Affairs, Network Contracting Office 7 of 325 Folly Rd in Charleston, SC 29412 intends to solicit proposals to enter into a firm-fixed price contract to provide Transitional Housing Community Based Health Care Services to Homeless Veterans . The contractor shall provide all resources necessary to accomplish Transitional Housing Community Based Health Care Services within catchment of the Ralph H. Johnson VA Medical Center located at 109 Bee St. in Charleston, SC 29401 and surrounding CBOCS for an initial period of one (1) year beginning on December 15, 2019 with four (4) options to extend if successful results are achieved per VA Medical Center s evaluation. The contractor shall furnish services to beneficiaries from whom such care is specifically authorized by the RHJ Veterans Affairs Medical Center and who are homeless and many with substance abuse and/or other mental health diagnosis. This contract may include male or female beneficiaries with children and/or significant other with children. The solicitation package will be available for download on or about November 1, 2019. The applicable NAICS code is 624229 - Other Community Housing Services with a small business size standard of $15 Million. This solicitation will be un-restricted and not Set-Aside for Small Business concerns. The solicitation reference is will be 36C24719R0152 Transitional Housing and should be referenced in ALL correspondence/ inquiries. All information for this solicitation will be posted on the Federal Business Opportunities (FBO) website at www.fbo.gov. No hard copies of the solicitation will be provided.
