The purpose of this work is to install a new Veeder-Root RF TLS-450 ATG with wireless sensors or industry standard equivalent for the 25K fuel oil tank located underground in the loading dock B service court in front of the cooling tower that supplies fuel oil to the main boiler plant room BC115, and repair the tank inspection sump which has been breached and fills with rainwater from the surrounding area in the service court, and the disconnection and removal of the existing plant Veeder-Root TLS solid state TLS-350 ATG located in room BC115. Note: All equipment and parts and miscellaneous items demolished will be disposed of off-site by the contractor. The VA retains possession of the old TLS-350 ATG monitor and tank probe. The contractor will perform the following work and furnish and install the following equipment listed below. Pressure wash, clean and dry the tank sump. Remove any necessary piping and risers from the sumps to get access to the entry fittings where the sump is leaking. Clean the silicone from around the entry sleeve and cut the sleeve flush with the sump wall. Using pneumatic tools, sand the sump wall around each pipe and clean smooth for new entry fitting. Clean the sanded area around each pipe entry and make necessary repairs to the sump wall using fiberglass sheets and fiberglass before installing the entry fitting. Install fiberglass entry fittings around all the pipe entering the sumps through the sump walls. Reconnect all the piping and risers that were removed from the sump if necessary. Remove the dirt and debris down to the gravel between the sump lid opening and the manhole skirt so that the surface water will drain into the tank pit through the gravel. Hydrostatic test the sump to insure there are no leaks. Remove existing ATG system for the tank located in room BC-115. Saw cut, break and remove concrete from the UST sump to the corner of the cooling tower slab. Excavate a trench 12" x 24" in excavated area and place (1) ¾" conduit in the bottom of the trench. Cut an entry hole in the side of the UST sump for the ¾" conduit and run the conduit into the sump using a fiberglass entry boot so seal the conduit to the sump to prevent leaking. Exit the trench at the cooling tower slab by turning a joint of ¾" conduit up connecting it to the slab with the required explosive fittings attached to the conduit. Backfill the excavated area up to 6" from existing grade using the material removed from the excavation and compact. Drill dowel hole in the existing concrete down each side of the trench ever 16" and place a dowel in each hole. Pour 6" of 4000 PSI concrete with fiber in the excavated area and finish. Run approximately 200' of conduit up approximately 8' and across the bottom side of the cooling tower to the building and up to the room where the tank monitor will be mounted. Mount the tank monitor on the wall using existing 120-volt AC power located within 5' feet of the tank monitor. Core Drill a hole through the concrete wall that the tank monitor is mounted on aligned with the adjacent wall in the stairs area. Run conduit through the drilled hole and across the wall to the ceiling in the entry way in front of the elevators. Run conduit from the boiler control panel in the boiler room to and through the back wall into the ceiling area. Pull (1) Cat-6 cable approximately 300' through the conduit from the boiler control panel to ceiling area, across the drop ceiling into the hall, down the hall toward the elevators, across the drop ceiling in front of the elevators into the conduit going from room with stair to the tank monitor. Pull (2) shielded cables from the UST tank sump to through the new conduit to the tank monitor. Install New RF TLS-450 head unit or industry standard equivalent (customer is responsible for providing 120-volt power) Install Communication board and Input / Output Relay board into head unit. Install RF Wireless Receiver and wire to head unit. Install Probe, sump sensor, and secondary leak sensor into tank. Install RF Wireless Transmitter mounting Brackets. Install wireless Transmitters with battery backups for probes and sensors. Install Remote Alarm and acknowledgment panel. Program and Test RF TLS-450 head unit or industry standard equivalent with wireless probe and sensors. Run new ¾ NPT conduit for CAT6 cable from TLS-450 to boiler management system controller in boiler room. Provide training of VA boiler plant and Safety GEMS coordinator in the operations, troubleshooting etc. of the RF TLS-450 or industry standard equivalent system for the tank. Provide Georgia EPD ATG certification for the current year of this contract. 2. Background: The old TLS-350 system provided monitoring of three 25K UST. Under the CHP project two of the 25K UST are being removed leaving a total of one 25K UST to be monitored. The old boilers have been replaced with 4 new modern boilers. The need for an extra 50K gallons of fuel oil is no longer required as one 25K tank is now adequate to meet the boiler plant requirements per VHA-1810. The present Veeder-Root TLS 350 system has been in service for over 25 years and is due for replacement. The ongoing construction over the past two years in the service court has rendered the present Veeder-Root TLS 350 ATG system inoperable and the cost to repair the current obsolete system is not as cost affective compared to doing a complete system replacement for the future needs of the VA Health Care System. 3. Performance Period: This shall be a firm, fixed price to be completed 30 business days after contract is awarded. 4. Type of Contract: This shall be firm fixed price contract. B. CONTRACT AWARD MEETING The Contractor shall not commence work on this SOW until the Contracting Officer has conducted a kick- off/ Pre-Con meeting or as advised by the COR that the kick- off/ Pre-Contract meeting is waived. C. GENERAL REQUIREMENTS 1. For every task, the contractor shall identify, in writing, all necessary sub tasks (if any), associated costs by task, together with associated sub milestone dates. The contractor's sub task structure shall be reflected in the technical proposal and detailed work plan. 2. All written deliverables shall be phrased in lay person language. Statistical and other technical terminology shall not be used without providing a glossary of terms. 3. Where a written milestone deliverable is required in draft form, the VA will complete their review of the draft deliverable within (5) calendar days from date of receipt. The contractor shall have (10) calendar days to deliver the final deliverable from date of receipt of the government s comments. D. SPECIFIC MANDATORY TASKS AND ASSOCIATED DELIVERABLES Description of Tasks and Associated Deliverables: The contractor shall provide all necessary material to complete the replacement of the Veeder-Root TLS-350 system with a new Veeder-Root RF 450 ATG system with wireless sensors or industry standard equivalent for one 25K tank. This includes all items listed below and these items and other deliverables, including the product manuals, documentation, field record drawings, close-out documentation, and product warranties, as directed by the COR, that record all significant changes and work to install the Veeder-Root RF TLS-450 system or industry standard equivalent for Tank #3. Repair tank sump as described in this SOP and hydrostatic test to ensure sump is sealed. Remove existing ATG system for the tank located in room BC-115. Install New RF TLS-450 head unit or industry standard equivalent (customer is responsible for providing 120-volt power) Install Communication board and Input / Output Relay board into head unit. Install Wireless Receiver and wire to head unit. Install Probe, sump sensor, and secondary leak sensor into tank. Install Wireless Transmitter mounting Brackets. Install wireless Transmitters with battery backups for probes and sensors. Install Remote Alarm and acknowledgment panel. Task One: The Contractor shall provide a detailed work plan and briefing to the VA project team, which presents the contractor's plan for completing the task order. The contractor's plan shall be responsive to this SOW and describe, in further detail, the approach to be used for each aspect of the task order, as defined in the technical proposal and the SOW prepared by the COR. E. EVALUATED OPTIONAL TASKS AND ASSOCIATED DELIVERABLES Optional Task One: [ NOTE: SUBJECT TO IRM APPROVAL REQUIREMENTS] SCHEDULE FOR DELIVERABLES The Contractor shall complete the Delivery Date column in Attachment A for each deliverable specified. 2. Unless otherwise specified, the number of draft copies and the number of final copies shall be the same. 3. If for any reason any deliverable cannot be delivered within the scheduled time frame, the Contractor is required to explain why, in writing, to the Contracting Officer. This includes a firm commitment for when the work shall be completed. This notice to the Contracting Officer shall cite the reasons for the delay and the impact on the overall project. The Contracting Officer will then review the facts and issue a response in accordance with applicable regulations. G. CHANGES TO STATEMENT OF WORK Any changes to this SOW shall be authorized and approved only through written correspondence from the Contracting Officer. A copy of each change will be kept in a project folder along with all other documentation and deliverables for the project. Costs incurred by the Contractor, through the actions of parties other than the Contracting Officer, shall be borne by the Contractor. H. REPORTING REQUIREMENTS 1. The contractor shall provide the Task Order Project Manager (TOPM) with daily written progress reports. These are due to the TOPM by the second workday throughout the project's duration. The TOPM is required to provide weekly progress reports to the Contracting Officer s Representative (COR) by the fifth workday of each calendar week. 2. The progress report shall cover all work completed during the preceding week and shall present the work to be accomplished during the subsequent week. This report shall also identify any problems that arose and a statement explaining how the problem was resolved. This report shall also identify any problems that have arisen but have not been completely resolved with an explanation. I. TRAVEL- N/A J. GOVERNMENT RESPONSIBILITIES The government will provide access to the work area, a material storage and fabrication area, 120 Volt electrical power, and all work permits in a timely manner. The COR will provide related information of the existing work in place and the general conditions of the work area to the CO as needed. K. CONTRACTOR EXPERIENCE REQUIREMENTS The contractor must have a minimum of five years of experience working with the installation, repair and replacement of RF wireless systems for monitoring industrial storage systems to include but not limited to underground fuel storage systems. L. CONFIDENTIALITY AND NONDISCLOSURE It is agreed that: 1. The preliminary and final deliverables, all associated working papers, application source codes, and other material deemed relevant by VA which have been generated by the Contractor, in the performance of this task order, are the exclusive property of the U.S. Government and shall be submitted to the Contracting Officer after the task order. 2. The Contracting Officer will be the sole authorized official to release verbally or in writing, any data, draft of deliverables, the final deliverables, or any other written or printed materials pertaining to this task order. No information shall be released by the Contractor. Any request for information relating to this task order, as requested by the contractor, shall be submitted to the Contracting Officer for response. 3. Press releases, marketing material, or any other printed and electronic documentation related to this project shall not be publicized without the written approval of the Contracting Officer. CONTRACTOR PERSONNEL SECURITY REQUIREMENTS This language is still in draft form. The OI Service Center is reviewing alternative sources for background investigations. All the Contractor s employees who require access to the Department of Veterans Affairs computer systems shall be the subject of a background investigation and must receive a favorable adjudication from the VA Office of Security and Law Enforcement prior to contract performance. This requirement is applicable to the Contractor s subcontractor personnel that require the same access. Position Sensitivity The position sensitivity has been designated as Low Risk. Background Investigation The level of background investigation commensurate with the required level of access is minimum. Contractor Responsibilities The Contractor shall prescreen all personnel requiring access to the computer systems to ensure that they maintain a U.S. citizenship and can read, write, speak, and understand the English language. The Contractor shall submit or have their employees submit the required forms (SF 86 or SF 85P, SF 85P-S, FD 258, Contractor Fingerprint Chart, VA Form 0710, Authority for Release of Information Form, and Optional Forms 306 and 612) to the VA Office of Security and Law Enforcement within 30 days of receipt. The Contractor, when notified of an unfavorable determination by the Government, shall withdraw the employee from consideration for employment under the contract. Failure to comply with the Contractor personnel security requirements may result in termination of the contract. The VA Office of Security and Law Enforcement will provide the necessary forms to the Contractor or to the Contractor s employees after receiving a list of names and addresses of their expected on- site personnel. Upon receipt, the VA Office of Security and Law Enforcement will review the completed forms for accuracy and forward the forms to the Office of Personnel Management (OPM) to conduct the background investigation. The VA Office of Security and Law Enforcement, who are adjudicating the results of the background investigations received from the OMB, will notify the Contracting Officer and Contractor of the results. Upon being notified of a favorable determination, the Contracting Officer may issue a Notice to Proceed to the Contractor. H. REPORTING REQUIREMENTS 1.
