Purchase request: 652-24-3-617-0037 Description of Action: This acquisition is conducted under the authority of the Multiple- Award Schedule Program (41 U.S.C. 152(3) and 40 U.S.C. 501). Order against: FSS Contract Number: V797D-70087 Name of Proposed Contractor: A-dec Inc Street Address: 2601 Crestview Drive City, State, Zip: Newberg, OR 97132 Phone: 800-547-1883 Description of Supplies or Services: The estimated value of the proposed action is $614,905.70 This requirement is to provide A-dec dental chairs, and installation service to include dental chairs with delivery system and assistant s instruments that are integrated in the chair. Identify the Authority and Supporting Rationale (see below and if applicable, a demonstration of the proposed contractor s unique qualifications to provide the required supply or service. FAR 8.405-6(a)(1)(A): An urgent and compelling need exists and following the ordering procedures would result in unacceptable delays: FAR 8.405-6(a)(1)(B): Only one source is capable of providing the supplies or services required at the level of quality required because the supplies or services are unique or highly specialized: This Item was specifically specd by the A/E Design Firm for the Basis of Design (BOD) for the Fredericksburg dental suite. The A-dec dental chair model 511 is the only item that can be integrated into the already constructed floor designs. No other chair will work with the specific constructed infrastructure that will accommodate the specific bolt pattern, plumbing and electrical infrastructure that is integral to the installation of the A-dec dental chairs. FAR 8.405-6(a)(1)(C): In the interest of economy and efficiency, the new work is a logical follow-on to an original Federal Supply Schedule order provided that the original order was placed in accordance with the applicable Federal Supply Schedule ordering procedures. The original order must not have been previously issued under sole source or limited source procedures. FAR 8.405-6(b): Items peculiar to one manufacturer: A patent, copyright or proprietary data limits competition. The proprietary data is: These are direct replacements parts/components for existing equipment. The material/service must be compatible in all aspects (form, fit and function) with existing systems presently installed/performing. Describe Why You Believe the Order Represents the Best Value. Consistent with FAR 8.404(d) to aid the contracting officer in making this best value determination: Pursuant to VAAR 808.002 (VA Specific Priorities of Use of Government Supply Sources), A-DEC, Inc. FSS contract (V797D-70087) was found on GSA Schedule 65 II C under Category C40 (Dental Equipment and Supplies) and therefor being considered a Mandatory Source of Supply which was already found to be fair and reasonable. Describe the Market Research Conducted among schedule holders and the results or a statement of the reason market research was not conducted. A search was conducted on the e-library website, Mandatory VA Schedule 65 II C, Category 40 (Dental Equipment and Supplies) revealing that the A-dec, Inc. contract (V797D-70087) was listed and upon review of the contract it was determined that the requested dental chair, delivery system, and assistant s integrated instruments, were all available.
