Western North Carolina VA Healthcare System Charles George VA Medical Center, Asheville, NC Statement of Work NxStage System One S 1.0 Background The Charles George VA Medical Center (CGVAMC) is seeking replacement of two NXS Systems due to end-of-life replacement mandates. As part of the acquisition, the medical center will also acquire fluid warmers, upgrade kits, and stands as part of the system. 2.0 Scope The CGVAMC is seeking a Firm Fixed Price contract vehicle to procure two NXS Systems, fluid warmers, upgrade kits, and stands. 2.1 This contract is all-inclusive. Prospective offerors should read the requirements carefully to ensure a complete understanding of the contract requirements to ensure their proposal includes all expenses and costs associated with performance related to the contract. The Government will not reimburse offerors due to a failure on the offeror s part to consider all costs associated with performance on this requirement. 2.2 Business Hours All tasks associated with this order shall be completed during the CGVAMC s normal business hours, Monday through Friday from 8:00am 4:30pm excluding Federal holidays unless otherwise coordinated with the Contracting Officer s Representative (COR) or Surgery Staff. The remaining Federal Holidays for calendar year 2023 are as follows: Monday, May 29, 2023 Memorial Day Monday, June 19, 2023 Juneteenth National Independence Day Tuesday, July 04, 2023 Independence Day Monday, September 04, 2023 Labor Day Monday, October 09, 2023 Columbus Day Friday, November 10, 2023 Veterans Day Thursday, November 23, 2023 Thanksgiving Day Monday, December 25, 2023 Christmas Day Any other day determined by the President of the United States as a Federal Holiday. 3.0 Specific Tasks The contractor shall deliver two NXS Systems, fluid warmers, upgrade kits, and stands per the attached quote. The CGVAMC will return the old system to the vendor as trade-in. 4.0 Invoicing & Payment The Contractor shall submit invoices monthly in arrears, covering the products and/or services performed under this contract for the period covered. All invoices shall be submitted electronically to the Austin Finance Center. Invoices shall contain the following information: Contract Number Purchase Order Number Name of Vendor, address, and telephone number Invoice Number Period Covered Total Price (A)Functions to be performed, Continuous Renal Replacement Therapy (B) Performance required, or characteristics 1. Treatment data must be permanently storable and retrievable 2. Network / System Protected 3. Compatible with System EMR (CPRS) 4. Freestanding 5. Mobile / Easy Maneuverability 6. Wi-Fi capable 7.
