The purpose of this notice is to gain knowledge of potential qualified sources and their business size classification (HUBZone, 8(a), small, small disadvantaged, small veteran owned, or service-disabled veteran owned small business) relative to NAICS 562910 and Brand Name or Equal Too. Consistent with Public Law 109-461 VA's Veterans First Contracting Program , first priority would be given to SDVOSB and VOSB concerns, should there be sufficient firms available to compete under a set aside if the requirement goes to solicitation. If insufficient veteran owned and small business response is received to establish the basis for setting this action aside for other than large business participation, then the action will be advertised as full and open competition, in which case, all interested parties responding shall be eligible to bid. Nature and/or Description of the Action Being Processed: The contractor shall provide all labor, parts, travel, tools, material, equipment, supervision, and technical expertise necessary for the removal and disposal of hazardous materials such as: Asbestos, Lead, mold, and minor flooring repair work at the VA Medical Center, Hampton, Virginia 23667. Period of performance is May 1, 2022, to September 30, 2022. Services shall be provided in accordance with the terms and conditions listed herein. The Contractor will supply all transportation, labor, parts, materials, tools, specialized equipment, sufficient to support the Hampton VA Medical Center s need for a Task Order to provide abatement, repair, and disposal services for areas containing or effected by Asbestos, Lead, and Mold at the request of the Hampton VAMC Contracting Offices Representative (COR). The purpose of this requirement is to conduct asbestos, lead, mold and minor post remediation constructions and repairs through the contract performance period. The service area for this task order will be throughout real property of the Hampton VA Medical Center including the Transitional Resident Homes, local and connected Veteran s Cemetery, and Community Based Outpatient Clinics. The contractor is to safely isolate, abate, remove, and dispose of all hazardous waste materials including, but not limited to: Asbestos Containing Materials, Lead Based Paint, mold remediation debris, and mold contaminated materials. Upon remediation of the hazardous materials the contractor will, at the request of the COR, perform repairs on the abatement area to return the area to safe working condition. The contractor shall also perform any air sampling and monitoring required by the type of abatement being performed. Maintenance and Repairs. Once task has been completed, the contractor will be responsible for all inspections including performing a final inspection with the Contracting Officer s Representative. Contractor will be tasked to remove and return furniture back in its original location. Contractor will protect furniture, employee, and government property in the work area. The contractor will be tasked to identify any potential friable asbestos and flaky lead-based paint and any possible water penetrations that will, or have, form(ed) bacterial growth. If the contractor encounters any bacterial growth in the immediate work area, the contractor will remediate and seal with a mold inhibitor, and where feasible, when directed by the Medical Center, perform repairs to prevent/remediate the source of water intrusion. Once the interior and exterior investigation is completed, the contractor will be tasked to remove all debris, clean interior spaces, and all surfaces where work has been conducted. Contractor will utilize an HEPA VAC to minimizes any potential dust from being airborne, and wet wipe work and adjacent areas. Upon completion of this task, the contractors will provide a final cleaning upon inspection approval of the Medical Center Industrial Hygienist. During performance of work, Critical barriers will be installed to prevent the possibility of dust and exposure controls will be established as required by the COR. The contractor will remove all required barriers and interim life safety measures and infection control element upon completion of the work and after approval with final inspection by the COR. The contractor will ensure all work is conducted in accordance with OSHA, EPA, State and VA Directives. Coordination. The Engineering Service (ES), Safety Office, Industrial Hygienist will serve as the focal point and point-of-contact (POCs) for this contract. Personnel can be reached at 757-722-9961 extension 2143 or 757-592-5772. The contractor must pre-coordinate and preschedule work with the POCs. These tasks can be conducted and accomplished during normal duty hours (Monday-Friday 08:00AM-4:30PM). When work is required to be conducted outside of normal duty hours the schedule shall be pre-approved by the VA COR. The workflow sequence for requirements performed in this task order follows: When notified by the COR about a task requiring completion by this task order, the contractor will arrange a site visit to the area designated by the VA COR and perform an assessment of the requirement identified by the VA COR. The contractor shall be flexible and maintain availability to perform a site visit within 5 business days of notification of a task item by the VA COR. The contractor shall provide and maintain availability and special pricing to perform immediate or emergency services when specifically requested by the VA COR. For purposes of this contract immediate shall mean work starting 3 business days from notification of an emergency task requirement by the VA COR. The contractor will then prepare a proposal including detailed quote, timeline, and work summary including performance of the task identified in the assessment and all safety, inspection, and sampling requirements to perform the task in accord with the performance work statement. The contractor will generate the proposal within 3 business days of the site assessment. The VA COR will review the quote and work summary, coordinate, and seek approval for the task item internally to the Medical Center and provide authorization to proceed with the task item. The contractor will maintain the ability to start all approved tasks within 5 business days of approval by the VA COR, unless otherwise approved by the VA COR. Specifications and Specific Requirements. The following specifications will apply: All work will be accomplished in-accordance with applicable Life Safety and OSHA codes and regulations. Additionally, the Department of Veteran Affairs guidelines and policies will be adhered to. The Contractor will provide all transportation, materials, parts, tools, specialized equipment, labor, supervision, and technical expertise to conduct remediation and construction. This is a turn-key task order with the Contractor providing all the tools, equipment, parts, materials, and labor for each task item. The Contractor will respond to the Hampton VA Medical Center and contact the COR before accomplishing any work. Prepare the area for work and prepare to the post work area for air sampling. Coordinate all tasks with the Medical Center Industrial Hygienist. Accomplish invoicing in-accordance with the formal contract. The contractor will be tasked to track any possible water or steam leaks throughout the area of task. If the contractor encounters any bacterial growth in the immediate work area, the contractor will remediate and seal with a mold inhibitor. Once the interior investigation is completed, the contractor will be tasked to remove all debris, clean interior walls, and all surfaces where remediation and installation has been conducted. Contractor will utilize an HEPA VAC to minimizes any potential dust from being airborne, and wet wipe work and adjacent areas. The contractors will provide a final cleaning upon approval of the medical center industrial hygienist. Environmental air sampling will be conducted by the contractor to ensure staff or patients are not being exposed to environmental stressor for Asbestos, Lead, Mold, Total and Respirable Dust. The contractor will ensure all work is conducted in accordance with OSHA, EPA, State and VA Directives. Upon completion of the task, A copy of the environmental air sampling results will be provided to the Medical Center Industrial Hygienist within five working days of completion of the project. The contractor shall perform environmental air monitoring and provide their own industrial hygienist for monitoring/testing/analysis to include any potentially hazardous substances within the task scope. Prior to each monitoring/testing/analysis the contractor must provide a cost estimate for the requested tasks(s). Projected quote is preferred. Service shall be provided within two working days of accepted quote. At the request of the COR contractor shall make repairs to flooring, ceiling piping, and interior to wall piping which contains asbestos pipe lagging and asbestos flooring abating the affected area where pipe lagging is adjacent to asbestos elbows. The contractor shall perform crawlspace soil abatement work, encapsulation, or other task requirements indicated by the VA COR. Depending on the assessment identify bacterial growth at various location AC units, ceiling tiles, furniture, HVAC systems duct work, flex duct work, crawlspace, and mechanical room. The VA Medical Center maintenance/repair/renovation shall conform to the requirements of the Joint Commission on Accreditation of Health Care Organizations (JCAHO) Environment of Care Standards and other applicable codes and standards. When required to complete a task, the contractor shall move and return all furniture, furnishing, storage racks, file cabinets, desks, chairs, wall décor, and other applicable items to the original position following abatement as identified and at the request or identification by the COR. The contractor shall be responsible for providing all work site protective barriers and site control devices. This includes, but is not limited to: protective fences, protective tapes, protective storage, and protective signage. The contractor shall be responsible for providing all necessary traffic control (i.e., street blockages, traffic cones, and flagman) as part of this task order whenever necessary to complete the task requirements under this task order. The cost of this requirement shall be built into the task costs proposed prior to initiating the discrete task item. Proposed traffic control methods shall be submitted to the COR for final approval. No street or access way shall be completely closed to traffic without prior approval from the COR. The contractor shall take all precautions to ensure that no damage to private or public property will result from their operations. If such damages occur, the contractor shall make all necessary repairs and/or replacements at not cost to the government. Contractor will provide their own environmental air sampling where required and provide a copy of the sampling results to the medical center industrial hygienist no later than five days of completion of project. All samples shall be analyzed by a laboratory accredited to perform analysis of non-violate spores under AHERA. All other laboratory analysis required to be performed as part of this contact shall be done in accordance with EPA approved methods, within AIHA guidelines as applicable, OSHA guidelines as applicable, and good professional practice for industrial hygiene and environmental sampling. Invoicing shall be performed in-accordance with the formal contract. The contractor shall include in each task cost ay pre-abatement activities including pre-abatement meetings(s), inspections(s), notifications, permits, submittal approvals, regulated area preparations, emergency procedures and arrangements, and standard operating procedures. The contractor is required to be located within an area permitting a maximum of a four (4) hour response time to the location listed above. The Contractor s office/location must be operational upon award of the contract and throughout the duration of the performance and must be open for business from 7:30 a.m. to 4:30 p.m., Monday through Friday, excluding federal holidays and weekends. Work is to be accomplished in accordance with VA MASTER CONSTRUCTION SPECIFICATIONS SECTION VA 02 82.11,13.13,13.19,13.21,13.31,13.41, and VA 02 83.33.13 . SCHEDULING OF WORK Before commencement of work under this contract, the contractor shall confer with the Contracting Officer s Representative (COR), as well as the building occupant, and agree on a sequence of procedures; means of access to premises and building; space for storage of materials and equipment; delivery of materials and use of approaches; use of corridors, stairways, elevators, and similar means of communications; location of partitions, eating spaces, and restrooms for contractor employees; and the like. Delivery of materials and equipment shall be made with a minimum of interference to Government operations and personnel. The work shall, so far as practicable, be done in definite sections or divisions, and confined to limited areas which shall be completed before work in other sections or divisions is begun. Most work will be performed in occupied areas. The contractor shall take all precautions to ensure that no damage will result from his operations to private or public property. All damages shall be repaired or replaced by the contractor at no cost to the Government. The contractor shall be responsible for providing all work site protective barriers and site control devices. This includes but is not limited to: protective fences; protective tapes; and protective signage. The contractor shall be responsible for providing all necessary traffic control, such as, street blockages, traffic cones, flagman, etc., as required at no additional cost to the Government. At the beginning of each working day, the contractor shall notify the Contracting Officer s representative of the location(s) of work to be accomplished that day. All temporary outages of any utility services required for the performance of work shall be scheduled with the Contracting Officer s representative no less than fourteen (14) working days in advance of such outages. Request for power outages must be in writing. The contractor shall prepare and furnish a weekly report. The Contracting Officer will approve the format and content of the report. The report will be broken out by Medical Center. A copy of the Medical Center s open projects will be provided to the Contracting Officer s representative located at the applicable facility, and a copy will be provided to the Contracting Officer. Information contained in the status report should be the award date, start date, dollar amount, required completion date, actual completion date, schedule progress and actual progress percentage, and comments. The contractor is required to submit a progress chart and construction cost breakdown as required in Schedule of Work Progress . Most work will be in occupied areas. The area wherein work is to be performed under this contract may be occupied by Government services during the construction period. The contractor shall have access to that portion of the area within which work is to be performed. The movement of contractor personnel, equipment, materials, and tools shall be confined to this area so as not to interfere with ongoing operations in the work areas. Badging may be required if necessary. CONTRACTOR QUALITY CONTROL The Contractor Quality Control (QC) Plan, with which the contractor proposes to implement the requirements of FAR Clause 52.246-12, entitled Inspection of Construction , shall identify personnel, procedures, instructions, records, and forms to be used. After contract award, and prior to commencement of work under this contract, the contractor s Quality Control Plan shall be received, reviewed, and formally accepted in writing by the Contracting Officer. Failure to execute the Quality Control Plan shall result in withholding of funds from progress payments in accordance with FAR Clause 52.232-5, entitled Payments Under Fixed-Price Construction Contracts . In general, the contractor s quality control plan must address how they intend to assure that their construction complies with the requirements of the contract plans and specifications. Coordination Meeting: Prior to the start of work, the contractor shall meet with the Contracting Officer or designated representative and discuss the contractor s quality control system. During the meeting, a mutual understanding of the system details shall be developed, including the forms for recording the CQC operations, control activities, testing, administration of the system for both on-site and off-site work, and the interrelationship of contractor s inspection and control with the Government s Quality Assurance. Minutes of the meeting shall be prepared by the contractor and signed by both the contractor and the Contracting Officer. The minutes shall become a part of the contract file. There may also be occasions when subsequent conferences will be called to reconfirm mutual understandings. The Quality Control Plan shall include as a minimum, the following: A description of the quality control organization, including chart showing lines of authority and acknowledgement that the CQC staff shall report to the project manager or someone higher in the contractor s organization. The qualifications, duties, responsibilities, and authorities of each person assigned a quality control function. A copy of the letter to the Quality Control Manager, signed by an authorized official of the firm, which describes the responsibilities and delegates the authorities of the Quality Control Manager, shall be furnished. Procedures for scheduling and managing submittals, including those of subcontractors, offsite fabricators, suppliers, and purchasing agents. Control testing procedures for each specific test performed on this contract will be submitted and approved prior to the start of construction. (Laboratory facilities will be approved by the Contracting Officer). Reporting procedures, including proposed reporting formats and distribution of the documents. The Government reserves the right to require the contractor to make changes in his CQC Plan and operations as necessary to obtain the quality specified. These changes must be submitted to and approved by the Contracting Officer. Notification of Changes: The contractor shall notify the Contracting Officer in writing of any proposed changes. Proposed changes are subject to acceptance by the Contracting Officer. Quality Control Organization QC System Manager: The contractor shall identify an individual, within his organization, who shall be responsible for overall management of QC and have authority to act in all CQC matters for the contractor. The QC System Manager shall be approved by the Contracting Officer and replaced if the QC System Manager is unable or unwilling to perform his duties as prescribed. Personnel: A staff shall be maintained under the direction of the QC System Manger to perform all quality control activities. The actual strength of the staff during any specific work period may vary to cover work phase needs, shifts, and rate of placement. The personnel of this staff shall be fully qualified by experience and technical training to perform their assigned responsibilities and shall be directly hired by and work for the prime contractor. Minimum Qualifications: Inspectors shall have working knowledge of general construction. The major areas of construction are electrical, mechanical, site work, masonry, finish work, and carpentry. The inspectors may have expertise in one or two areas and may inspect those areas when that type of work is ongoing. Minimum Staffing: Progress shall be comprehensively inspected at least daily, and these inspections shall be documented. SUBMITTALS: The QC organization shall be responsible for certifying that all submittals are in compliance with the contract requirements. CONTROL: Contractor quality control is the means by which the contractor assures himself that his construction complies with the requirements of the contract plans and specifications. The controls shall be adequate to cover all construction operations, including both on-site and off-site fabrication, and will be keyed to the proposed construction sequence. Quality control includes, as a minimum, the following functions: All submittals are submitted in a timely fashion. The submittals are approved. The supplies that are delivered are the same as the ones on the submittal. The supplies are in the proper condition when delivered. The supplies are stored properly. The construction equipment is correct and meets contract requirements. Testing provisions are reviewed and testing equipment and personnel are available and correct. All tests are performed at the proper times and in the proper places. All test reports meet contract requirements. The workers are cognizant of the required level of workmanship. Inspect each area of work to ensure the preparation for the work is correct. Inspect each feature of the work to ascertain that no deficient work is covered up by succeeding work. Inspections shall continue throughout the contract. Document all inspections. The documentation covers both conforming and defective work. All deficiencies are corrected. Develop procedures to ensure that deficiencies do not recur. Develop a punch list for the completion inspection. Government officials are notified at the proper times of inspections or tests that are required. TESTS: Testing Procedures: The contractor shall perform tests specified or required to verify the control measures are adequate to provide a product which conforms to contract requirements. The contractor shall procure the services of an industry recognized testing laboratory or establish an approved testing laboratory at the project site. Documentation: Records: The contractor shall maintain current records of quality control operations, activities and tests performed, including the work of suppliers and subcontractors. The records shall be on the Daily Construction Quality Control Report and indicate a description of trades working on the project, the number of personnel working, the weather conditions encountered, any delays encountered, and acknowledgement of deficiencies noted, along with the corrective action taken on current and previous deficiencies. In addition, these records shall include factual evidence that required activities or tests have been performed, including, but not limited to, the following: Type and number of control activities and tests involved; Results of control activities or tests; Nature of defects, causes for rejection, etc.; Proposed remedial action; and Corrective actions taken. Content: These records shall cover both conforming and defective or deficient features and shall include a statement that supplies, and material incorporated in the work have been inspected and comply with the contract. Two (2) legible copies of these records shall be furnished to the Contracting Officer s Representative daily. SAFETY ASSURANCE Preconstruction Safety Meetings: Representatives of the contractor shall meet with the Contracting Officer or his/her representatives prior to the start of repair, alteration, or construction activities for the purpose of reviewing the contractor s safety and health programs and discussing implementation of all safety and health provisions pertinent to the work to be performed under the contract. Preconstruction Safety Meetings shall be coordinated by the Contracting Officer or his/her representative. The contractor shall be prepared to discuss, in detail, the measures he/she intends to take to control any unsafe or unhealthy conditions associated with the work to be performed under the contract. This meeting may be held in conjunction with the preconstruction conference, if so directed by the Contracting Officer or his/her designated representative. The conduct of this meeting is not contingent upon a general preconstruction meeting. The level of detail for the safety meeting is dependent upon the nature of the work and the potential inherent hazards. The contractor s principal representative(s), the general superintendent, and his/her safety representative(s) shall attend this meeting. Contractor Responsibility: The contractor shall assume full responsibility and liability for compliance with all applicable regulations pertaining to the health and safety of personnel during the execution of work and shall hold the Government harmless for any action on his part or that of his employees or subcontractors, which results in illness, injury, or death. Contractors are required to report any accidents or injuries to the Contracting Officer Representative within 24 hours of the incident. Inspection, Tests, and Reports: The required inspections, tests, and reports made by the contractor, subcontractors, specially trained technicians, equipment manufacturers, and others, as required by this contract, shall be furnished in accordance with the terms of the contract. Materials and Equipment: Special facilities, devices, equipment, clothing, and similar items used by the contractor in the execution of work shall comply with the applicable regulations. Government Provided Tools, Materials and Equipment. The Federal Government will not provide any tools, parts, materials, and equipment. This is a turn-key requirement with the vendor providing all necessary tools, parts, materials, and equipment. Period of Performance. This contract is intended to cover the period of May 1, 2022, to September 30, 2022. This will be a full-service contract which includes the following: contractor will follow manufacturer s recommendations/specification for asbestos, lead, mold abatement, and minor repair. Other Station Rules and Guidelines. The Contractor will adhere to the following station rules. All work will be accomplished to OSHA Standards. All Dept. of Veteran Affairs, Federal, Commonwealth of Virginia laws, regulations and codes will be complied with. This is a hospital environment. The Contractor and his personnel are expected to be appropriately dressed (preferably uniformed) and to conduct themselves professionally. This is a federal facility as well as a medical center. As such, there is NO tobacco usage allowed inside any federal building or on the grounds except in designated areas. The POC will brief the Contractor as to designated smoking areas prior to commencement of the contract. Failure to follow these regulations can result in removal of the worker or Contractor from the facility and/or a US Magistrate s ticket and fine. Parking will be in designated lots. Consult with the POC as to allowable parking and material drop-off. The Contractor will be responsible for ensuring that all the employees and other personnel visiting the work site have and are using the appropriate Personal Protective Equipment (PPE) that addresses the hazards found at the site. The Contractor will be responsible for the upkeep, issue, and training for the PPE used and maintaining all required documentation. Since the incidents of September 11, 2001, the Federal Government has begun to implement new security measures to help protect federal facilities. The Contractor will provide to the POC a list of employees expected to work on this contract. This list will be on company letterhead and contain the employees name, driver s license number, address, and nationality. This list will be verified and signed by a corporate officer. These personnel are subject to a background check and fingerprinting to be eligible to receive a security badge. The U.S. Department of Veterans Affairs and the U.S. Department of Health and human Services requires that all Contractor personnel coming on this facility to complete the Veterans Health Administration s Privacy Policy Training prior to coming on the facility. The Contractor will submit the certificate, to be completed, at the end of the Microsoft PowerPoint Presentation for each person assigned to this contract who comes on this facility to work. A company roster certifying course completion may be substituted for individual certificates. Security badges will be picked up from the Police Office in Building 135 and will be returned daily by 3:30 PM unless other arrangements are made with the POC and the Police Office. Loss Badges can result in administrative fees and fines not to exceed $50.00 USD to replace the loss badge. Interested and capable respondents are requested to provide the information below.
