Part# Description Laerdal 212-00050PMC SimMan 3G Preventative Maintenance Customer Site Includes 1 service (on site), full refurb/replacement of consumables, some bladders, pad sets, refurb of IV arm skin/veins, inspect compr/ lbox, upgrade SW, cleaning, final inspection, documentation of findings/ recommendations. Part #212-61450 included at no charge. Laerdal 212-00050EXW1 SimMan 3G Extended Warranty Extended warranty includes materials, manufacturing or workmanship issues, return to bench service only, packaging materials for return to Laerdal, shipping costs for repairs (ground freight from/to customer's facility). Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used: This is a new requirement request is for maintenance support services for the Laerdal SimMan 3G System serial# 21224154586 located at the Durham Medical Center. The contractor is to provide all transportation, labor, parts, tools, specialized equipment, unlimited software/hardware updates and support, supervision and technical expertise for corrective maintenance in accordance with the Laerdal SimMan 3G System Service Maintenance Agreement. The vendor must be able to provide certified trained technicians by the original equipment manufacturer (OEM) to assist in the maintenance of these systems. Additionally, the vendor must be able to provide OEM parts and software necessary to maintain maximum reliability and uptime at no additional cost to the Durham VAMCs. Description of market research conducted and results or statement why it was not conducted: A Sole Source Statement from the manufacture has been attached that states the manufacturer is the sole source provider of the Laerdal SimMan 3G System Maintenance Support Plan. All maintenance support is required to go directly through the manufacture Laerdal. Best value is determined on the risk assessment of the third-party repair cannot provide certified services for proprietary equipment. The Durham VAMC provides operational services to its patients with brand name Laerdal equipment. The equipment requires a periodic maintenance support to ensure consistent operations in the hospital. The manufacture is the only authorized distributor that provides periodic maintenance support for its equipment required from the manufacture. Any unauthorized maintenance and/or modifications will void the warranty and provide unnecessary risk to the equipment s operation that are critical to the services rendered to our veterans. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief.
