Department of Veteran Affairs PSL Classic Hand-Held (Portable) Slit-Lamp Biomicroscope (Brand Name or Equal) Request for Information (RFI) The Department of Veterans Affairs (VA) is issuing this RFI in accordance with FAR 15.201(e). This RFI is issued for the purpose of conducting capabilities and market research. Accordingly, this RFI constitutes neither a Request for Proposal (RFP) nor a guarantee that one will be issued by the Government in the future; furthermore, it does not commit the Government to contract for any services described herein. Do not submit a proposal. The Department of Veterans Affairs is not at this time, seeking proposals or quotes and therefore will not accept, review, or evaluate unsolicited proposals or quotes received in response hereto. This notice is not to be construed as a commitment on the part of the Government to award a contract, nor does the Government intend to pay for any information submitted as a result of this request. The Government does not reimburse respondents for costs associated with submission of the information being requested or reimburse expenses incurred for responses to this RFI. The information provided may be used by VA in developing an acquisition strategy and Product Description (PD). Any information submitted by respondents to this RFI is strictly voluntary; however, any information received shall become the property of the Government and will not be returned to the respondent. Interested parties are responsible for adequately marking proprietary, restricted or sensitive information contained in their response. The Department of Veterans Affairs (VA), VISN 5 Network Contracting Office, is seeking sources that can provide Portable Slit Lamps for the Huntington VAMC, Huntington, WV. BACKGROUND: Medical facility in need of three portable slit lamps with case. LOCATION: Huntington VAMC, 1540 Spring Valley Dr, Huntington, WV 25704 ITEM INFORMATION: Product Number Product Description Units of Measure Quantity SL0KEPSLFG Keeler PSL Classic Slit Lamp - with Case Each 3 ESTIMATED DELIVERY: 30 Days ARO STATEMENT OF WORK: 1.0 General 1.1 Background The mission of the eye clinic of the Huntington Veterans Affairs Medical Center is to provide care for its veteran population; in this task, multiple devices are daily employed. 1.2 Objective The PSL Classic hand-held (portable) slit-lamp biomicroscope is used in patient-care to examine structures of the eye and its adnexae, usually of patients who are chair- or bed-bound. 1.3 Scope of Work Same as above. 1.4 Period of Performance From date of award until end-of-life of item. 1.5 Place of Performance This device is to be used on-site, in the eye clinic of the Huntington Veterans Affairs Medical Center. 1.6 Applicable Documents All applicable documents (e.g., price quotation, sole source letter, and technical specifications) are attached. 2.0 Specific Requirements/Tasks The contractor will coordinate with the Veterans Affairs Medical Center department of biomedical engineering, to service all on-site installation of hardware and software, as well as to provide any on-going support within the frame of their warranty-period.
