STATEMENT OF WORK Mattress Replacement 1. Description: Our goal is to assess all the CLC beds for risk assessment tool compliance. This has been done on the current mattresses on the CLC and most have failed. Therefore, we are currently requesting 25 mattresses with a liner and protective coating to replace current mattresses that do not meet compliance. Additionally requesting 3 wound care mattresses for the CLC. 2. Scope of Work: Beds are to be delivered and installed at the Beckley VA Facility. 3. General Requirement: The following Sizewise Mattresses are required below. Model: 7770-N9G3580, NP 9 G Series Mattress, (size 35x80x7) Model: 7770-NPT303580, NPT3 Mattress, (size 35x80x7) Model: 7770-ALB3537, 35 MAP Liners, (standard) 4. Service and Maintenance: N/A 5. Installation: Beds are to be delivered and installed. Destination pricing. 6. Background: Since 2001, guidance has been issued from various groups including the VA National Center for Patient Safety (NCPS), The Federal Drug Administration (FDA) and oversight bodies including Center for Medicare Service (CMS) regarding patient safety risks pertaining to bedrails. Mitigation strategies may include accessories or devices. Evaluation of equipment, environmental and patient factors are critical to ensure patient safety. Recently a facility submitted a request for bed purchase, citing issues with compliance with CMS F-tag 700. The VISN requests input and confirmation that CLC beds in the facility comply with FDA guidelines and minimize risks for patient harm. Risks may be inherent in the bed frame/rail; or when in combination with a mattress/overlay or bed accessory. The intent of F-tag 700 and F-tag 909 guidance is to reduce resident harm and life-threatening entrapment associated with hospital bed systems. Risk to the resident can be exacerbated through improper bed installation and maintenance, improper bed rail to bed frame. 7. Delivery: 60 days ARO to Beckley WV, VA MEDICAL CENTER, 200 Veterans Ave. Beckley, WV. 25801. 8. Period of Performance: March 21, 2022 July 30, 2022 9. Contract Type: Firm-Fixed Priced Contract 10. Evaluation Factors: 1. Price: Acceptable quote, vendor provided distribution authorization letter and had the lowest price, Unacceptable quote, vendor did not provide the distribution letter and did not have the lowest price.
