STATEMENT OF WORK GENERAL INFORMATION The Washington DC medical center is in need of Renal hemodialysis Liquid Drum solution. The SB-1020 liquid acid concentrate series is formulated to be mixed in conjunction with Renal Systems BC-1 Series. The solution is a crucial supply requirement to mixing the powdered solution to continually provide treatment to dialysis patient. Item Required: SB 1020 DRUM (16 Drum Monthly) -SOLUTION, HEMODIALYSIS, LIQUID ACID, LIQUID, DRUM, SODIUM 139 MEQ/LITER, CALCIUM 2.5 MEQ/LITER, MAGNESIUM 0.75 MEQ/LITER, POTASSIUM 2.0 MEQ/LITER, CHLORIDE 105.5 MEQ/LITER, ACETATE 4.0 MEQ/ L, BICARBONATE 35.00 MEQ/LITER, RENASOL 2. Scope of Work: The contractor shall provide agreed-to quantities of Solution, on the agreed-upon schedule during the term of the contract. Timing is critical to assure efficient use of the solution within their storage life cycle. The vendor will NOT have to pick up the empty drums. 4. Performance Period: The period of performance is December 2021, to December 2022, with option for four (4) years. 5. Type of Contract: Firm-Fixed-Price 6. Place of Performance: No on-site performance is required of the contractor. The required materials are sequestered at their location and shipped to the VA facility on the agreed quarterly schedule.
