The Pathology and Laboratory Medicine Service requires urine toxicology testing to continue to be performed by Friends Laboratory, Inc. This contract will cover invoicing for urine toxicology testing through Option year 1 from October 1, 2019 through September 30, 2020 and will include the following tests: a. EIA Fentanyl screen (urine) b. Fentanyl LC/MS Confirmation c. Thin Layer Chromatography S2 Screen for Benzodiazepines d. Enzime Immuno Assay (EIA) for Benzodiazepines e. Benzodiazepines (LC/MS) f. Buprenophine confirmation (LC/MS) g. Opioids (LC/MS) h. Amphetamines i. Other tests as required. 2. Specimen pick-up will be requested, as needed. Specimen containers will not contain any patient Personally Identifiable Information (PII) and specimens will be identified only by accession number. 3. Results will be sent to two lab employees via email for entry into Vista computer system.
