SUBJECT: The Department of Veterans Affairs is conducting market research in order to provide a Medical Physicist for Diagnostic/Interventional Radiology services for the Washington DC VA Medical Center. The government anticipates awarding a Firm Fixed Price contract that will provide the support services listed within the below attachment 1. OBJECTIVE: See attachment 1. CONTACT INFORMATION: Any questions related to this Sources Sought shall be directed to Amy Ritchey at amy.ritchey@va.gov regarding Capabilities Statements or any other proprietary information relative to this Sources Sought shall be submitted via email to amy.ritchey@va.gov no later than 2:00 pm EST on 05/02/2019 ATTACHMENT 1 Washington DC VAMC Statement of Work DC VETERANS AFFAIRS MEDICAL CENTER CONTRACT SERVICE MEDICAL PHYSICIST FOR DIAGNOSTIC/ INTERVENTIONAL RADIOLOGY STATEMENT OF WORK SCOPE OF WORK: The Contractor shall provide an American Board of Radiology certified, Medical Physicist to supplement the internal Radiation Safety Officer. The Medical Physicist shall: Be required to conduct an annual compliance survey of all radiation producing equipment here at the DC Veterans Affairs Medical Center (DC VA MC) with the skin entrance sticker where applicable. The most current list of the aforementioned equipment can be found in Section V of this statement of work. Conduct radiation compliance testing after all repairs affecting the imaging chain (generator, X-ray tube, collimator, image receptor) of a unit or any other modification that makes re-testing necessary. These compliance tests may need to be performed on urgent basis including weekends to assure that the device operates within compliance. Provide compliance reports that include measurements obtained to determine compliance, indicate whether the tested parameter passed or failed, and recommend corrective action for a failed test. Provide analysis of the current and possible future quality assurance programs through the use of audits, reports and tests and recommend improvements for implementation. Participate in the quarterly review/ optimization of computed tomography, digital radiography, computed radiography, and fluoroscopy protocols as well as reviews of patient and personnel exposure records. Support new and or ongoing installations of radiation producing equipment. This includes providing consultation/ direction as applicable and necessary regarding site prep, room design, and shielding requirements. He/she shall oversee shielding installations and verify that the installed shielding is acceptable. He/ she shall make shielding calculations and evaluations for the facility as needed. As needed, he/she will participate in pre-purchase evaluations and acceptance testing. Assure compliance with the Joint Commission Sentinel Alert 47 and will design and analyze patient dose evaluations/ dose reconstructions annually and as requested by the Contracting Officer s Representative (COR). These reviews shall be conducted in accordance to industry recognized standards and best practices. Conduct Quality Assurance (QA) reviews at least annually, including Magnetic Resonance Imaging, mammography, computed tomography, fluoroscopy, linear accelerator and other x-ray producing imaging equipment. The report shall document whether the program is complaint with the American National Standard Institute (ANSI) standards, American Association of Physicist in Medicine (AAPM) reports (TG 66 and TG 142), manufacturer s specification and ACR guidance such as the Technical Standards for Diagnostic Medical Physics Performance Monitoring of CT Equipment . Report shall include a detailed listing of QA parameters reviewed, indication of whether the tested parameter passed or failed, and recommendations for corrective action for a failed test. Attend quarterly meetings of the Radiation Safety Committee and Sub Committee and will periodically brief executive leadership on program status. Conduct an analysis of staff-submitted fluoroscopy procedure times for the purpose of identifying high risk procedures. Advise the hospital s Occupational Dose Program and generate detailed reports as it relates to the program. Provide at least one (1) radiation safety and patient dose reduction in-service per year to the clinical staff, who use radiation producing equipment. Assist the Chief of Radiology in completing physician fluoroscopy credentialing. Fluoroscopy safety training shall be conducted at least biannually prior to recertification/re-credentialing for fluoroscopy privileges. Provide business hours phone consultation as needed. Provide emergency response/on call as needed. Perform work that meets all National, State, and VA codes, requirements, and standards to include, but not be limited to, The Joint Commission, The Office of the Inspector General, the Nuclear Regulatory Commission, the National Health Physics Program, and the American College of Radiology. Respond to survey new or malfunctioning equipment and emergency consultation within one business day of the request. AREAS TO BE SERVICED: Washington DC VA Medical Center 50 Irving St. Northwest Washington, DC 20422 Southern Prince George s County Community Based Outpatient Clinic 5801 Allentown Road Camp Springs, MD 20746 TASK FREQUENCY AND INSTRUCTIONS This contract shall require multiple visits to the facility during the contract period. All visits shall be coordinated with Washington DC VA Medical Center staff and the COR. All written reports shall be submitted by the Medical Physicist to the appropriate parties, including the contract s COR, within thirty (30) days of completed testing or evaluation. The Medical Physicist shall also spend one (1) to two (2) days per month at the Washington, DC facility to supplement and support the on-staff Radiation Safety Officer as necessary and appropriate to meet the requirements set forth in this scope of work. All compliance testing shall be done in accordance with the Center for Devices and Radiological Health (CDRH), Mammography Quality Standards Act (MQSA), and the American College of Radiology (ACR) guidance. Shielding calculations, evaluations, and/or verification surveys shall be conducted in accordance with National Council on Radiation Protection and Measurements (NCRP) guidance and requested by the COR. Emergency requests shall be responded to and addressed within two business days of the requirement. OPTION YEARS The contract shall be set forth for one base year and four option years. The agency may solicit bids from other vendors before the renewal of an option year. QUALIFICATION OF VENDOR Vendor personnel must be trained and certified to test the specific equipment listed in the contract and must submit a certificate of training to the COTR prior to commencing work on this contract. Furthermore, vendor must be knowledgeable of the required procedures and tests to be performed on the specific equipment and must be able to advise Radiology Service of the updates and changes that should be implemented. The Medical Physicist provided by the Contractor shall possess all credentials and/ or permits required by any and all applicable regulatory bodies, including but not limited to: the Nuclear Regulatory Commission, the National Health Physics Program, the American College of Radiology, and the Joint Commission to perform the work of a Health Physicist. PAYMENT All invoices shall be submitted using the OB-10 electronic invoicing system. DOCUMENTATION REQUIREMENTS All reports shall be provided in a timely manner, no later than thirty (30) days following the performance of the test on the equipment and received by the COR of the Contract in the Radiology Service. LOCATION OF AUTHORIZED VENDOR The vendor is to provide specific names and contact information of staff assigned to support this contract. The vendor will also provide the correct name of company for billing and a local address. COMPLIANCE WITH VETERANS AFFAIRS RULES AND REGULATIONS Contractor Personnel Shall be Required to: Comply with all security and confidentiality requirements of all federal/state/local laws and regulations. Comply with Veterans Affairs polices in regards to proper use of government equipment and sexual harassment. Complete all mandatory VA Training. PROPERTY DAMAGE The contractor shall take all necessary precautions to prevent damage to any government property. The contractor shall report any damages immediately and shall be assessed current replacement costs for property damaged by the contractor, unless corrective action is taken. Any damaged material (i.e., trees, shrubs, lawn/turf, curbs, gutters, sidewalks, etc.) will be replaced in a timely manner or corrected by the contractor with like materials, at no extra cost to the government, upon approval of the Contracting Officer. IDENTIFICATION, PARKING, SMOKING, CELLULAR PHONE USE AND VA REGULATIONS: The contractor's employees shall wear visible identification at all times while on the premises of the VA property. It is the responsibility of the contractor to park in the appropriate desig nated park ing areas. Information on parking is available from the VA Police-Security Service. The VA will not invalidate or make reimbursement for parking viola tions of the con tractor under any con ditions. Smoking is prohibited inside any buildings at the VA. Cellular phones and two way radios are not to be used within six feet of any medical equipment. Posses sion of weapons is prohibited. Enclosed containers, including tool kits, shall be sub ject to search. Violations of VA regulations may result in a citation answer able in the United States (Federal) Dis trict Court, not a local district state, or municipal court. COMPLIANCE WITH OSHA BLOODBORNE PATHOGENS STANDARD In the event that a contractor, or contractor representative works within an environment where there is potential for contact with a bloodborne pathogen (though not anticipated), the contractor shall comply with the Federal OSHA Bloodborne Pathogens Standard. The contractor shall: Have methods by which all employees are educated as to risks associated with bloodborne pathogens. B. Have policies and procedures which reduce the risk of employee exposure to bloodborne pathogens. C. Have mechanisms for employee counseling and treatment following exposure to bloodborne pathogens. D. Provide appropriate personal protective equipment/clothing such as gloves, gowns, masks, protective eyewear, mouthpieces for the employee during performance of the contract.
