Stephens City CBOC Agency Specific Requirements 12-3-18 Community Based Outpatient Clinic At the time of initial submission, the Lessor must prepare Design Intent Drawings (DIDs) for the leased space that conforms to this RDP. The Government shall review the Lessor's DID's to determine that the Lessor's design meets the requirements stated in this RDP. Only after the Government approves the DIDs will the Lessor be released to proceed with development of working/construction drawings. The general layout of the clinic shall incorporate spaces defined in the SEPS Program for Design (PFD) provided separately, while incorporating as many of the design strategies/principles, module functional layout and dimensions, finishes, typical equipment layouts and the separation of patient and staff flows within the space as described in the Patient Aligned Care Team PACT Space Module Design Guide dated June 2015 (Primary care). Due to space constraints, the design will result in a Hybrid PACT model. Provide Sample layouts in the offered space to reflect best fit of required spaces within the physical configuration of the offered building. Particular attention should be given to maximizing the separation of patient and staff circulation through the clinic. Exam rooms, consult rooms, team workspace rooms and procedure rooms shall comply with the typical room layouts as shown in the design guide. Equipment to be accommodated within each type of room shall comply with typical room layouts, and be as described in the Outpatient Clinic Equipment Guide List, also provided separately. Provide any required support in wall construction for wall mounted equipment and casework as shown on the typical room layouts. If feasible, Exam, consult and procedure Rooms shall utilize sliding door frames to minimize the impact of door swings on corridors and the functionality of the exam room useable floor space. Maximize the use of natural light in all patient and staff areas. Mental health space, telehealth exam rooms and all specialty consult/exam rooms be incorporated into the Clinic layout. Submit sample best fit drawings indicating how the program space would be laid out in the offered building for approval. Room numbering, interior signage and wayfinding in the clinic shall be consistent with existing building room numbering and interior graphics / signage if a system already exists. If no interior graphics already exist with which to conform, then interior signage, room numbering and graphics will be provided by the VA in accordance with signage Design Guides found on the VA Technical Information Library. Where appropriate, lessor shall provide a ground mounted, illuminated, horizontal monument sign to identify the Outpatient Clinic in accordance with signage Design Guides found on the VA Technical Information Library. Lessor shall provide foundations and electrical power as necessary. Base shall be concrete or masonry and shall be compatible with building design and landscaping scheme. VA will furnish message layout, content, and colors for the monument sign. Signage must comply with local municipality s codes and specifications. Careful consideration of the location of monument signs shall be taken to avoid sight triangle encroachment. Lessor shall coordinate with state and local agencies for placement of signage defining the route to the clinic from surrounding major thoroughfares. A flagpole shall be provided at a location approved by the Contracting Officer. Flagpole must extend at least 30 feet above the ground and shall be equipped with rope and hardware. The Government will provide flags. This requirement will be waived if determined inappropriate by the Government. Exterior lighting shall be provided to illuminate the flags at night. Automatic switching for light fixtures shall be provided. All exam rooms, consult rooms procedure rooms, team work rooms, and shared appointment room partitions will be designed and constructed to provide a sound transmission coefficient (STC) of STC 40. Group Therapy rooms will be constructed to provide a sound transmission coefficient of 45. If compatible with the exam room design and layout, provide sliding door frames at corridor entrances to exam rooms, consult room, and procedure rooms as shown in typical room layouts. Minimum width of patient doors will be 42 Inches for bariatric wheelchair clearance. All door hardware shall be compatible with the VA medical center s Corbin Russwin hardware, (7-pin, small format). Locks shall have removable cores. Locks will be provided with construction cores keyed alike during construction. Provide two cores per door to VA Medical Center for keying into the VA Medical center s grandmaster keying system. If the entrance to the clinic is accessed directly from the outside, provide a vestibule with double set of automatic doors to provide an airlock. Automatic doors shall operate by motion sensors. Sensors shall be adjusted to minimize door activation by pedestrian traffic walking by the outside or inside of the entrance to the clinic. Main exterior doors leading to the Government demised area shall be sliding glass doors with a 48 inch opening, automatically opened through the use of motion sensors. These doors shall be equipped to break outward in case of an emergency. The opening dimensions and operations shall conform to the governing building, fire safety, accessibility for the disabled, and energy codes and/or requirements. Provide handrails, corner guards and wall guards in all corridor walls as described in the technical requirements. Handrails shall comply with ADA and guard rails shall be of matching construction and color and mounted at a height to protect walls from damage from carts, wheel chairs and other wheeled equipment. Room and corridor dimensions shall follow the PACT Design Guide for minimum clear distances. Corridors shall not be less than 6 foot clear unless otherwise noted. IT / Data room construction requirements and HVAC requirements are described in technical specifications for TER/TR rooms. Finished ceiling heights in exam rooms, consult rooms, tele health rooms, procedure rooms, team work spaces, group rooms, shared appointment rooms, waiting rooms shall be minimum of 9 6 . If these ceiling heights cannot be attained a waiver must be requested and approved. Ceiling heights less than 9 0 will not be acceptable. Ceilings shall have a minimum noise reduction coefficient (NRC) of 0.60 throughout the Government-demised area. Prior to closing the ceiling, the Lessor shall coordinate with the Government for the installation of any items above the ceiling. Physical security requirements for the clinic shall comply with the guidance outlined in the Request for Proposal (RFP). If the Lessor s design cannot comply with these physical security requirements, provide a list of any standards that cannot be accommodated along with Lessor s recommendations for alternate standards as the basis for request / approval of a waiver. The offered property shall comply with the requirements for an ENERGY STAR label as noted in section 2.09 of the Request for Lease Proposals Handwashing sinks will be provided in all exam rooms, consult rooms and other spaces as shown in typical room layouts shown in PACT design guide. Provide wall and base cabinets. DIRTT wall or Hermann Miller modular sink cabinets and wall cabinets or an acceptable approved equivalent are required. Sink base counters shall be solid surface. Provide certifications that the building is free of lead, radon, asbestos, mold and all other hazardous materials and that the building water quality and air quality meet required standards. HVAC equipment shall provide design air conditions for all occupied spaces in accordance with VA HVAC standards for type of room. See HVAC Design Manual for VA facilities, Chapter 6 Application, Room HVAC Design Criteria FIRE PROTECTION FOR NEW CONSTRUCTION the Offeror shall provide a written statement from a licensed fire protection engineer that the building(s) fully complies with the fire protection and life safety requirements for a community based outpatient clinic The new building shall be protected throughout by an automatic fire sprinkler system designed in accordance with the National Fire Protection Association (NFPA) 13, Installation of Sprinkler Systems. When an electric fire pump is provided to support the design of the fire sprinkler system for the building, a secondary power source shall be provided to the fire pump by a standby emergency generator or another means acceptable to the Government. A fire alarm designed and installed to meet the requirements of the applicable codes and ordinances adopted by the jurisdiction in which the building is located (Minimum compliance with NFPA 72) The landlord should provide the monthly testing for the Fire Extinguishers, Fire and Smoke doors as well as emergency lights and provide the VA with the test results. All windows shall be weather-tight. Operable windows that open shall be equipped with locks. Off-street, ground level windows and those accessible from fire escapes, adjacent roofs, and other structures that can be opened must be fitted with a sturdy locking device. Windows accessible from fire escapes must be readily operable from the inside of the building. Window Blinds. All exterior windows shall be equipped with window blinds in new or like new condition. The blinds may be aluminum or plastic vertical blinds, horizontal blinds with aluminum slats of 1-inch width or less, solar fabric roller shades, or an equivalent product preapproved by the Contracting Officer. The window blinds shall have non-corroding mechanisms and synthetic tapes. Color selection will be made by the Contracting Officer. Floor surfaces A. All adjoining floor areas shall be: Of a common level not varying more than 1/4 inch over a 10-foot horizontal run in accordance with the American Concrete Institute standards, Non-slip, and, Acceptable to the Contracting Officer. B. Underfloor surfaces shall be smooth and level. Office areas shall have a minimum live load capacity of 50 pounds per ANSI/BOMA Office Area square foot plus 20 pounds per ANSI/BOMA Office Area square foot for moveable partitions. Storage areas shall have a minimum live load capacity of 100 pounds per ANSI/BOMA Office Area square foot including moveable partitions. A report showing the floor load capacity, at no cost to the Government, by a registered professional engineer may be required. Calculations and structural drawings may also be required. C. Ambient Noise Control. Ambient noise from mechanical equipment shall not exceed noise criteria curve (NC) 35 in accordance with the ASHRAE Handbook of Fundamentals in offices, clinical areas, and conference rooms; NC 40 in corridors, cafeterias, lobbies, and toilets; NC 50 in other spaces. Noise Isolation. Rooms separated from adjacent spaces by ceiling-high partitions (not including doors) shall not be less than the following noise isolation class (NIC) standards when tested in accordance with ASTM E-336: Conference, exam rooms, team work spaces, consult rooms, classrooms, telehealth rooms, Mental Health Rooms NIC 40 Group Therapy Rooms NIC 45 4. Testing: The Contracting Officer may require, at no cost to the Government, test reports by a qualified acoustical consultant showing that acoustical requirements have been met. Utilities and Power for equipment Coordinate required utilities, power and supporting construction for VA and contractor provided equipment shown in the room specific equipment list and technical requirements shown. Power outlets for work stations shall consist of one quadplex (or two duplex) receptacle for each work station. Maximum 3 circuits per homerun and maximum 6 outlets per circuit. Coordinate height of outlets for wall mounted TV and other wall mounted equipment that requires a power outlet. Handicap accessibility - The design, construction, and alteration of facilities shall comply with local codes and ordinances. In addition, all VA facilities must comply with the Architectural Barriers Act Accessibility Standards (ABA-AS) as adopted by GSA and VA Program Guide PG-1813, "Barrier Free Design Guide." Interior Room Locking Requirements All interior doors shall be lockable. Keyed locks shall be compatible with Corbin Russwin 7 pin cores. Doors designated for electronic card reader access shall be FICAM and FIPS 201 compliant and compatible with current Martinsburg VAMC Hirsch/Identiv card readers, controllers and Velocity access control software.
