Question 1: Reviewing your Solicitation No. 36C24424Q0262 Slit Lamp and Table | Wilkes Barre, I noted CLIN 0010 - Reichert 11625B Minus Cylinder - Ultramatc Rx Master Phoropter.  This item is on SAC NX Contract 36C10G23D0019 and must be ordered under that contract. Answer 1: Contract line item 0010 - Reichert 11625B Minus Cylinder - Ultramatc Rx Master Phoropter has been removed from this solicitation through modification 0001. Question 2: I would like to confirm that the only items to be submitted in response to this solicitation are the items listed under E.2 52.212-1 Addendum to FAR 52.212-1 as listed on pages 44 and 45 of the solicitation. I would also like to confirm you do not need the entire SF 1449 issued as part of this solicitation. Answer 2: As stated on the 1449 above the signature block and within the addendum on pages 44-45, including the signed 1449 form is a requirement for submission. Any requests in the description of the 1449 or the addenda and any other location in the solicitation are required.
