Department of Veterans Affairs Medical Centers are in need for a Home Oxygen delivery and set-up to Veteran beneficiaries within the VISN-04 East catchment area. The contactor shall arrange and be responsible for providing Home Oxygen supplies and services to Veteran beneficiaries serviced by VA Medical Center Prosthetics and Sensory Aid Service (P&SAS). The VA has determined that this acquisition will be an SDVOSB set-aside based on recent market-research implemented by 38.U.S.C.8127. Under Federal Acquisition Regulation (FAR) 12.204 a written commercial solicitation will be issued under competitive procedures utilizing a Performance Price Tradeoff (PPT) Source Selection for the following; NAICS 532283; PSC W065 VISN-04 Veterans Affairs Medical Centers (VAMC) in Wilmington, DE, Philadelphia, Coatesville, Lebanon and Wilkes-Barre Prosthetic & Sensory Aids Services (PSAS) have a requirement for Home Oxygen (HO) services to be provided by a contractor. HO services will consist of providing home medical equipment leasing services for Oxygen concentrators and Oxygen tanks, as well as providing all necessary resources & services for the pick-up, delivery, installation of HO equipment and refilling of Oxygen tanks. The contractor will also be providing Respiratory Therapists to conduct initial and follow up visits with patients, which includes providing user-level education regarding the proper use of the equipment. See attached Statement of Work, Performance Work Statement. Supplies and Services are attached and are an estimated/realistic quantity that will be acquired for a period of 7 years (84 months) from date of award with a Requirements Contract FAR 16.503 (supply and service).
