B.4 STATEMENT OF WORK General: The scope of work shall generally consist of, but not necessarily be limited to, providing all labor, management, materials, tools, and equipment, required to identify all fire doors and (or) smoke doors located on a smoke barrier shown in the 2017 Life Safety Assessment drawings. The contractor will inventory, labeled, categorize, inspect, annotate deficiencies, and compile a report as described in Task 1. Under Task 2 the government may elect to repair deficient doors based on the price schedule. Under Task 2 the contractor shall test all access-controlled doors that act as fire doors for proper function or high security doors for function during fire alarm. Task 3, the contractor shall repair identified existing deficient fire doors that are also security (PIV Controlled) doors. The inspections and repair of doors will take place at the Wilkes Barre VAMC located in Wilkes Barre, PA. The contractor shall use the most recent version of National Fire Protection Association (NFPA) codes as applicable for the inspection (NFPA 101 / 2021edt, NFPA 80 / 2019edt, NFPA 105 /2019edt, and or any other NFPA codes by reference) Regular work hours are Monday through Friday 7:00AM to 4:00PM. After hours work must be approved by the Technical Point of Contact. This inspection contract shall include four (4) executable option years at the conclusion of a base year award. This will be a 5-year requirements contract. The estimated period of performance is 11/1/2023 to 10/31/2028. Contract Task: Task 1: Inspection of Doors. Using the WBVAMC 2022 Life Safety Assessment (LSA) drawings, Schedule A and the contractor shall validate and inspect approximately 576 doors to be inspected based on Schedule A and the drawings showing fire barriers, chutes, fire rated rooms and smoke barrier doors at all locations shown in the 2022 LSA, and Schedule A. The contactor shall add to the inventory any additional fire doors, chutes, smoke barrier doors and hazardous room doors to be inspected as needed and approved through the VAMC Safety Office. The contractor shall inspect all doors identified as fire and or smoke barrier doors in accordance to established protocols found in NFPA 80 /2019edt and NFPA 105 /2019edt. The contractor shall label all door frames with a vinyl sticker indicating inspection date and have a QR code specific to the door and linking to a database that will retain the specific information about the door. Database door information shall include the type of door, location (room information), new naming convention with cross reference of existing facility naming convention, inspection criteria, Pass/Fail, photos, information about failure/ deficiencies and recommendations on corrections. (e.g. minor repair maintenance to correct, correct screw holes, Repair by frame replacement, Repair by door replacement) The database shall be accessible via the internet and downloadable in to an excel file. Vendor must provide certification of inspector as well as proof of minimum of 5 years experience. The inspectors qualifications shall not be less than 5 years of experience installing and inspecting fire doors at least one member of the field inspection team shall be qualified to field label fire doors. The Competent Qualified Person inspecting the smoke doors and doors to hazardous areas shall have not have less than 5 years of experience installing and inspecting smoke doors and those which lead to rooms designated as Hazardous Areas. The inspector shall have completed training (i.e. Door Hardware Institute or equivalent) and experience to demonstrate competence in inspecting smoke doors and hazardous doors. Other atypical barriers at WBVAMC: The contractor shall inspect 3 West automatic accordion smoke door, Basement North Pharmacy window roll-down smoke barrier, Boiler plant sliding 90min fire door and Boiler plant cross corridor doors. Both boiler plant doors use fusible links, the links shall be replaced during the course of the inspection. The contactor shall not inspect elevator landing doors or cab doors. Temporary contractor installed 45min construction area ICRA doors do not need to be inventoried or surveyed. Doors located on a fire/smoke barrier maybe partially in a construction area and shall be inspected (e.g. electrical vault room has a 90min door one side of the door has electrical switch gear the other side is within a construction area) A yearly onsite inspection of the fire doors shall be completed within the dated period of performance listed on CLINs 0001,1001, 2001, 3001, and 4001. The contractor shall compile a report and submit the report in within 30 days from the conclusion of the inspection. The contractor shall provide two hard copies of the inspection report and also provide the report electronically in PDF format. The contractor developed database shall be maintained online for 6 months from date of award. Task 2: Inspection of Access Control system and controlled fire doors Using the WBVAMC 2022 Life Safety Assessment (LSA) drawings, Schedule A, and possibly the 2017 LSA the contractor shall identify doors that are PIV or elopement system controlled and act as fire door. The contractor shall inspect all access control doors (including PIV controlled and patient elopement) on a fire barrier or egress path (e.g. rated storage room, corridor, stairwell, or door to pubic way (exit to outside)) for proper operation in access control mode (normal day to day operation) and proper configuration when fire alarm is activated. The inspection shall also include verification that on fire alarm the security door either stays locked or unlock depending on type of door. Task 3: Repair of doors (Bid Addition) The contractor shall provide a price schedule for the following repairs. Repair Description (per item e.g. hole / hardware / gap adjustment / screw / door) Price List Remobilization to the site (lump sum) Â Hole in Frame less than 1/2 inch dia. (hole) Â Hole in Frame between 1 inch and 1/2 inch dia. (hole) Â Hole in Steel Fire Door less than1/2 inch dia. (hole) Â Hole in Steel Fire Door between 1 inch and 1/2 inch dia. (hole) Â Hole in wood Fire Door <1/2 inch dia. (hole) Â Add / replace missing screw hardware (replace add one screw) Â Add / replace Sweep (sweep) Â Add / replace Door Coordinator (coordinator) Â Add / Replace Door Hinges (all Hinges on door) Â Add / Replace Door Shims (gap adjustment) Â Inspect and Field Label Fire / Smoke Door add to inventory (per Door) Â The government may or may not elect to use Task 3 for repair of fire or smoke doors. Should the government elect to have the doors repaired under this contract the repair work will be done after the door inspection report has been issued, therefore there will be mobilization cost seen in price schedule above. This work associated will be issued by task order once cost, line items, and quantity of each repair line item are established. Prior to engaging in any repairs the contractor shall obtain written approval from the CO and the Government POC. Email is an acceptable form of written approval. Any work performed without CO and Government POC approval shall not be approved for payment and considered and Unauthorized Commitment. The Government reserves the right to obtain repair services from other vendors. This repair work is not included in the inspection cost but can be procured on separate order under this contract. CLINS 0002, 1002, 2002, 3002, and 4002 apply to this paragraph. The repair work shall commence 30 days after an agreement of line items and doors to be repaired is made by the contracting officer and the contractor.
