Page 1 of Page 2 of 2 STATEMENT OF NEED General information Title of Project: Top Door Alarm System Scope of Work: This package is to purchase the Top Door Alarm System in our Emergency Department. Top Door Alarm system is used as a safety measure to prevent suicide attempts while being treated in the Emergency Department. This system will be installed on 3 Doors in 3 different rooms of the ED. Background: The Lebanon VAMC has a brand name requirement to install a Top Door Alarm system in the Emergency Department. The Lebanon Staff is looking to have this system installed on 3 rooms and 3 separate doors. This system is used as a suicide prevention method. The Top Door Alarm System is currently used in the Behavioral Health Department as well as the other sections of the Emergency Department. Performance Period: The contractor shall complete delivery of all equipment and installation required under this SOW within 30 days of contract award. If the contractor proposes an earlier delivery date, and the Government accepts the contractor s proposal, the contractor s proposed delivery date shall prevail. Equipment delivery to the Government site shall not take place on Federal holidays or weekends unless directed by the Contracting Officer (CO) or a representative of Biomedical Engineering. Annual Certification, as per manufacturer specifications the system will require an annual inspection and recertification at a minimum of once annually by a qualified manufacturer inspector starting one calendar year from installation certification. Type of Contract: Brand Name required, Firm-Fixed Price Contract award meeting The contractor shall not commence performance on the tasks in this SOW until the CO has conducted a kickoff meeting, or the CO has advised the contractor that a kickoff meeting is waived. Specifications/Requirements of the Brand Name Required Top Door Alarm System: Materials per door, should include: 3 Each- Electrified Full Mortise Continuous Hinge Materials per Room, should include: 3 Each- Top Beam only entry door kits; customers to provide and install key cylinder and core Materials per system to include 1 each- Master j box, local display/ interface panel and hallway siren Travel and training for onsite support of staff Contractor shall provide all necessary equipment, Materials, and installation for Top Door Alarm System at Lebanon VA Emergency Department on 3 doors in 3 rooms. DELIVERY Contractor shall complete delivery of all equipment within 30 days of contract award. Packaging shall be marked to the attention of Biomedical Engineering and shall be delivered to the following address: Lebanon VA Medical Center Building 6, Warehouse 1700 S.
