Combined Synopsis/Solicitation This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 and supplemented with additional information in this notice. The solicitation number is 36C24423Q0158 and it is issued as a Request for Quotes. This announcement constitutes the only solicitation. Quotes are being requested and a written solicitation will not be issued. The NAICS code is 339114 with a size standard of 750 employees. Successful offer will be the one that provides the Lowest Price that is Technically Acceptable IAW the attached solicitation document instructions. The Lebanon VA Medical Center (LVAMC) has a brand name only requirement for three (3) Schick 33 Starter Kit, and three (3) HD Plus Heliodent Arms. The Schick 33 will provide diagnostic images while the Heliodent plus will function as the intraoral x-ray generator for digital imaging. These will be installed in the 2 new Dental Operatories on 17-5A.
