SOURCES SOUGHT NOTICE: The Department of Veterans Affairs, Network Contracting Office 4 (NCO 04), is conducting a market survey to identify potential sources for the rental of specialty mattresses and bariatric equipment for the Community Living Center (CLC) at the Butler VA Medical Center (VAMC), 325 New Castle Road, Butler, PA 16001. This announcement is issued for informational planning purposes only and does not constitute a solicitation nor does it restrict the Government as to the ultimate acquisition approach. Submitting information for this announcement is voluntary and VA assumes no responsibility for any costs incurred associated with the preparation of responses submitted as a result of this notice. No solicitation is available at this time. If a solicitation is issued it will be announced at a later date, and all interested parties must respond to that solicitation announcement separately from the responses to this announcement. Additionally, there is no obligation on the part of the Government to acquire any products or services described in this Sources Sought Notice. The purpose of this market research is to identify capable sources that are both experienced and qualified to provide rental of specialty mattresses and bariatric equipment for the Butler VA Medical Center. This equipment is used for the prevention and/or healing of pressure ulcers and the safe management of patients at the Butler CLC. CAPABILITY STATEMENT AND DOCUMENTATION: Companies are invited to provide their capabilities, experience, and knowledge in the provision of these services. COMPANIES MUST BE ABLE TO PROVIDE ALL EQUIPMENT RENTALS LISTED. Sources are asked to provide the following information: 1. Company information to include: Company Name, Company Address, Company Unique Entity ID (SAM), Company NAICS Code(s), Point of Contact Name, Point of Contact Phone Number, Point of Contact Email Address, Company Website (if available). 2. The North American Industry Classification System Code (NAICS) is 532490 Other Commercial and Industrial Machinery and Equipment Rental and Leasing, and the small business size standard is $35.0 Million for this type of effort. Based on this information, please indicate whether your company is a Service Disabled Veteran Owned Small Business (SDVOSB), Veteran Owned Small Business (VOSB), 8(a), HUB Zone Small Business, Women Owned Small Business, Small Disadvantaged Business (SDB), Small Business Concern, or Large Business. Prospective Contractors are reminded that in accordance with FAR 4.11 they shall be registered in the System for Award Management (SAM) database and shall complete electronic annual representations and certifications in SAM at the time an offer or quotation is submitted.
