This request is for information only and does not constitute a request for proposals. Submission of any information in response to this market survey is purely voluntary. The Government assumes no financial responsibility for any costs incurred associated to this request for capability information. The U. S. Department of Veterans Affairs (VA) desires to lease sufficient Rentable Square Footage (RSF) of real property space to yield up to approximately 14,935 Net Usable Square Feet (NUSF) in Huntingdon County, PA for use as a Community Based Outpatient Clinic (CBOC). Space must be contiguous and handicapped accessible, outside 100-year flood plain, and VA prefers space located on the first floor; space must be accessible by elevators if not on first floor. The Department of Veterans Affairs will consider existing space that complies with all current building, seismic, and ADA code requirements. On-site, dedicated parking for 100 vehicles is required. Delineated Area: Space within or fronting on the following boundaries will be considered: West: Intersection of Routes 305 and 22. 22 to Bridge Street, to Main Street, to Juniata Valley Pike (305) into the town of Petersburgh, PA.  Juniata Valley Pike North becomes King Street then turns into Greenwood Road (305), then becomes Shavers Creek Road Northeast (305), Greenwood Road (305/26) Northeast, Greenwood Road (305) Southeast across Belleview. Stay on 305 Southeast to 655/East Main Street. Take 655 North towards Reedsville. to 322 South. North: Take 322 South to 4013 West to Feguson Valley Road, turns into Furnace Road to McVeytown. Turn onto Water Street.  East: Turn on John Street heading Northeast, to 103 South. Continue Southwest on 103 to the town of Mattawana, PA. South: Continue on 103 Southwest to Croghan Pike in the town of Allenport, PA, which is State Route 522. Take SR 522 Croghan Pike to Shirley Street through the center of Mount Union, PA. Take Shirley Street to North Jefferson Street, to State Route 22 West. Follow State Route 22 West to Oriskany Road West, then North. Oriskany Road is also 655, merges back into State Route 22 West. Stay on State Route 22 West and North back to the intersection of Routes 305 and 22. Continue on 22 to Bridge Street. SDVOSB and VOSB firms are invited to provide information to contribute to the market research for this project. SDVOSB and VOSB firms must be registered in VA s Vendor Information Pages (VIP) site at https://www.vetbiz.va.gov and have the technical skills and financial capabilities necessary to perform the stated requirements. Only SDVOSB and VOSB firms are requested to submit a Capabilities Statement to VA for review. SDVOSB and VOSB firms may provide a Capabilities Statement if they are interested in participating in this project. A submission checklist and information sheet is provided below for firms to fill out and submit, which will serve as the firm s Capabilities Statement. Capabilities Statement Will Include: 1. Company name, address, point of contact, phone number, Dunn & Bradstreet number, and e-mail address; 2. Evidence of SDVOSB or VOSB status through registration at VIP (https://www.vip.vetbiz.gov/); 3. Evidence of ability to offer under NAICS Code 531120 and listing in the System for Award Management (www.SAM.gov), including a copy of the representations and certifications made in that system; 4. A summary describing up to three (3) projects of similar size and scope completed in the past five (5) years that demonstrate your company s experience designing, constructing, and managing Federal leased facilities or health care facilities relevant to the project described above. (3-page limit); 5. Evidence of capability to obtain financing for a project of this size. If desired, the company may also submit a narrative describing its capability. Although this notice focuses on SDVOSB and VOSB, we encourage all small businesses and other interested parties to respond for market research purposes. Expressions of Interest must be submitted to the Lease Contract Specialist by the date specified on Sources Sought Notice and must include the following information: Building name, address and location of available space Amount of offered space in Rentable Square Feet (RSF) and Net Usable Square Feet (NUSF) Building ownership information, brokers or legal representatives must show written acknowledgement and permission to represent the property Amount of parking available on-site The Government is limited by law (40 USC 278a, as Amended 10-1-81) to pay no more than the appraised fair rental value for space.
