The Wilmington VAMC is seeking SDVOSB/VOSB and any small or large company regardless of Socio-economic category that can provide the Brand OsteoMed Implant Block Sets requirement as spelled out in the Statement of Need (SON) and list of items below. The veteran business must be registered in betaSAM and certified in VETBIZ (VIP). The small or large businesses must be registered in betaSAM. All Vendors interested in, and with the capability and capacity to fulfill this requirement, are required to furnish the following minimum information in reply to the sources sought: Company Name, address, Government Business Point of Contact, telephone number and email address. Company DUNS number, CAGE Code, and Tax ID numbers. Identify any current Federal Supply Schedule, GSA Contracts held, if none then state that no FSS/GSA schedules are held. Certification from the manufacturer that they have permission to provide and install any systems or software. Are you a manufacturer, dealer, or distributor? New medical equipment will be required, all resellers, dealer, distributors must provide an authorized distributor letter from the Original Equipment Manufacturer (OEM). We currently use the vendors sets during surgery. This has created delays in the OR due to sterilization issues. OBJECTIVE: The Wilmington VAMC wishes to procure (2) OsteoMed Implant Block Sets. The period of performance requested for this acquisition is one-time purchase. The facility requests the ability to exercise up to zero option renewals.
