Contractor shall provide service including evaluation, manufacture, fitting and adjustment of Orthotic devices by its qualified Orthotist. Veterans who are eligible for Orthotic services may be provided with arch supports, shoe modifications, functional foot orthotics, depth inlay shoes, healing shoes, custom-made orthopedic shoes, and alternative footwear. Footwear and modifications must be prescribed by the appropriate VAPHS clinician and in accordance with the 1173 Handbook. The contractor will provide on-site services to the government by qualified Orthotists at the VA Pittsburgh Healthcare system, HJ Heinz Division, Monday thru Friday. In addition, the Contractor will provide on call emergency services as determined by a physician during off tour, evenings, weekends, and holidays. In addition, services will be provided upon request at the Contractor s commercial locations if the Veterans needs are best met there. The VAPHS credentialed physician will determine where the Veterans needs are best met. At least four locations are required, although more than four are preferred. At least two of the locations must be located within Allegheny County of Pennsylvania. The other locations must be somewhere within the following States/Counties: Pennsylvania: Beaver, Butler, Fayette, Greene, Indiana, Lawrence, Washington, or Westmoreland county Ohio: Belmont, Columbiana, Harrison, or Jefferson county West Virginia: Hancock or Marshall county SEE ATTACHED STATEMENT OF WORK FOR COMPLETE DETAILS AND REQUIREMENTS.
