This notice is for informational purposes only. The intent of this Sources Sought Notice 36C24421Q1135 is to identify any Small Businesses, mainly SDVOSB/VOSB vendors who can provide ENT Microscope, Exam Clinic and one (1) SMR Maxi Cabinet as listed in the attached Statement of Need. In addition, we are also seeking any Small Business manufacturers that have the capability of providing this system as well. This is neither a solicitation nor a promise of future work. Small Business, SDVOSB, and/or VOSB manufacturers authorized distributors are encouraged to submit your company s information and capability statement in response to this Sources Sought Notice. This notice does not constitute a formal Request for Quote (RFQ), nor is the government obligated to issues an RFQ. In addition, the Government does not intend to pay for any information provided under this notice. The Government is not obligated to notify respondents of the results of this survey. Potential vendors should respond with their ability to provide the following brand name: The Department of Veterans Affairs (VA), VISN 4, Network Contracting Office 4 (NCO 4), is seeking information and potential qualified sources capable of meeting the Statement of Work (SOW) for an Microscope, Exam Clinic and one (1) SMR Maxi Cabinet for the Lebanon VA Medical Center, 1700 South Lincoln Ave, Lebanon, PA 17042-7529. The information identified above is intended to be descriptive, not restrictive and to indicate the quality of the supplies/services that will be satisfactory. It is the responsibility of interested parties to demonstrate to the government that they are capable of providing the equipment or supply that fulfills the required minimum specifications mentioned above. STATEMENT OF NEED: Microscope, Exam Chair and SMR Maxi Cabinet Background: The Lebanon VA Medical Center (LVAMC) has a BRAND NAME requirement for one (1) 3-Step A-Series AXIS Microscope System, one (1) S 4000 Exam Chair and one (1) SMR MAXI Cabinet to be utilized in the ENT clinic for an additional ENT provider. Scope of Work: This equipment will be used for a newly hired second ENT provider and will provide continuity of care to the Veterans. The equipment required is identical to the current equipment that is currently being used in the ENT clinic. This is a Brand name requirement for one (1) 3-Step A-Series AXIS Microscope System, one (1) S 4000 Exam Chair and one (1) SMR MAXI Cabinet. This is required to guarantee continuity of care. This equipment must be the same brand and model to assure that staff is equally proficient, as well as standardizing capabilities should providers change/share rooms and need to utilize both sets of equipment. The ENT equipment shall be delivered to the Lebanon VAMC located at 1700 S. Lincoln Ave., Lebanon, PA 17042; Attn; Warehouse. BioMedical Engineering will need to perform a safety check and enter the equipment into the system for annual Preventive Maintenance (PM) checks. Placement in the LVAMC clinic shall be coordinated through the Contracting Officer Representative. Responses to this Sources Sought should include company name, address, point of contact, phone number, point of contact e-mail, DUNS Number, Cage Code, size of business pursuant to North American Industrial Classification System (NAICS) 339112, (size standard of 1000 employees). Respondents shall also answer the following questions: Size status and representations of your business, such as but not limited to: Service-Disabled Veteran Owned Small Business (SDVOSB), Veteran Owned Small Business (VOSB), HUBZone, Woman Owned Small Business (WOSB), Small Business (SB), Large Business, etc. Is your company considered small under the NAICS code identified under this Source Sought? Is your company the manufacturer, distributor, or equivalent source for the type of items required with the characteristics referenced above? If you accompany is a large business, do you have any authorized distributors? If so, please provide the name(s), telephone Number(s), Point(s) of Contact and size status of the businesses authorized to distribute your products. If your company is a small business and an authorized distributor/reseller for the types of items required with the characteristics referenced above, do you alter; assemble; or modify; the items requested in any way? If your company alters the items, state how and what is altered; assembled; and/or modified? Does your company have an FSS contract with GSA or the VA NAC or a GWAC contract holder with any other federal agency? If so, please provide the contract number. If your company holds a FSS GSA/VA NAC contract other federal GWAC contract holder, are the items you are providing information on available on your schedule/contract? General pricing of your products is encouraged. Pricing will be used for the purpose of market research only. It will not be used to evaluate for any type of award. Information provided by potential sources will be utilized by the Government to determine market availability and procurement strategy. An award of a contract will not result from this Sources Sought Notice.
