COATESVILLE VA MEDICAL CENTER | WOLF X-RAY Purpose and Objectives: The intent of this Sources Sought Notice is to identify potential businesses capable of providing Brand Name Wolf X-Ray Mobile Over the Table Radiation Barrier as listed in the below Statement of Need. This system will be delivered to the Coatesville VA Medical Center 1400 Black Horse Hill Road, Coatesville, PA. It is expected that NAICS Code 339112, Surgical and Medical Instrument Manufacturing will be used for this solicitation. Vendors to this Sources Sought Notice shall demonstrate the firm's ability, capability, and responsibility to provide the Brand Name Wolf X-Ray Mobile Over the Table Radiation Barrier below. Vendors shall include the following information: Business name, address, Point of Contact, Business size. Vendors shall indicate if they are the direct supplier of the items. All information is to be submitted via e-mail at Justin.Erdley@va.gov. Information provided will not be returned. All responses shall be in the English Language. This requested work is in direct compliance with department standard of operations for Safe Use of Fluoroscopic Equipment. Background: The Coatesville VAMC is requesting one adjustable mobile over the table Radiation shield for the Radiologist s safety when performing fluoroscopic procedures. Quotes shall include the following to be considered for award: 1) Disclosure of all Tort claims against vendor either pending or resolved regarding proposed system. 2) Registered in SAM.gov 3) Brochure or literature showing salient characteristic 4) Salient Characteristics: Mobile radiation shield provides a lead barrier in most any situation Barrier mount allows 360° rotation on vertical and horizontal planes, so it can be used in various working positions Semi-circular cutout fits closely to patient s torso for maximum shielding 6" offset pivot provides a maximum extension of 36" Adjustable Height 30 to 63 H x W X D 24 X 24 X 5 Delivery: One adjustable mobile over the table Radiation shield will be delivered to the Radiology department at the Coatesville VA Medical Center, Building 3-1st floor. Loading dock is behind building 1 Police Station.
