Zeiss Visual Field Machine Statement of Need Background: The Lebanon VA Medical Center (LVAMC) has a requirement for one (1) Zeiss Visual Field Machine, HFA3 Model 860 with Liquid Trial Lens, RelEYE, Kinetic, GPA, Heat Trk., Gaze Trk., Printer, and Table. The machine is to be utilized in a clinic setting in the York Outpatient Clinic. The LVAMC providers presently utilize this product in the Lebanon Facility, and the Lancaster Facility to service patients and we would like to maintain continuity in care, as well as allow for transmission of data between facilities with the use of Zeiss Forum which connects all equipment for immediate access of data regardless of where patient is cared for. Scope of Work: The Zeiss Visual Field Machine is utilized as a diagnostic tool to determine Glaucoma disease in Optometry patients. Contractor shall ensure any bi-directional communication is tested fully, allowing for sending and retrieval of data.
