In accordance with FAR 15.201(e), responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this notice. This notice is being issued in an attempt to determine the level of interest among all socioeconomic groups. Responses will assist VA planners in determining an appropriate acquisition and set-aside strategy which best fits VA requirements. The Erie Veterans Affairs Medical Center (VAMC) located at 135 East 38th Street, Erie, PA 16504-1559 is performing market research to determine if there is a sufficient number of qualified: (1) Service-Disabled Veteran Owned Small Business (SDVOSB); (2) Veteran Owned Small Business (VOSB); (3) Small or emerging small business firms (SB or other); or (4) large business (LB) firms to set aside a single award contract for ambulance/wheelchair/medi-taxi services. Attached is the DRAFT Performance Work Statement with a description of this requirement. The anticipated period of performance is to start January 23, 2022 for a total of five years. All interested firms who can meet the requirement should respond, in writing, and provide the following information: A. General Information: Information to be provided SHALL include, but not be limited to the following: Contractor s Information: Provide your firms contact information, such as: name and address of firm, phone number, e-mail address, DUNS number and the number of miles you are located from the Erie VAMC. Type of Business: whether Veteran Owned, Small, Small Disadvantaged, or Large (see second paragraph above). If you are considered a large business under NAICS 621910 ($16.5 million) are you familiar with subcontracting plans and goals? If you are a small business under NAICS code 621910 are you familiar with the limitations on subcontracting in FAR 52.219-14? Describe any of the following services your company can provide in the following areas: ALS/BLS Ambulance services Wheelchair Van services Medi Taxi Services Experience: Describe a minimum of 1 recent (within the past 3 years) and relevant (similar scope) experience as a contractor (specify prime or sub), where you have supported similar requirements. Indicate contract values and number of employees used in completing the contract tasks. Provide the number of contracts performing similar services, average value of contracts, value of largest contract, and contact numbers as applicable. Indicate experience working with VA. Describe your firm s capability to perform the tasks outlined in the PWS. Are you planning on subcontracting, if so what part(s) of the PWS? Do you provide services 24/7? What States do you currently have EMS operating licenses? Any other relevant information Existing Contracting Vehicles: If the respondent has a GSA FSS contract or another applicable contract, indicate so and include how your exiting contract is within scope of this requirement. 7) Are you currently enrolled in the Systems for Award Management (SAM) database? https://sam.gov/content/home Interested firms are reminded that in accordance with FAR 4.12, prospective contractors shall complete electronic annual representations and certifications required in the Systems for Award Management (SAM) database prior to award of a contract. B. Include any recommendations for the following: 1) Recommendations to improve or clarify the Draft PWS; what information can/should be added to the PWS? What information should be clarified within the Draft PWS? 2) What Government stated operating constraints or technical requirements limit you (the contractor) from developing a unique and innovative technical solution to the PWS? 3) Recommendations on the appropriate NAICS code for this requirement. 4) Any special licenses required to complete these requirements? NOTE: Contractor will be expected to transport Veterans within the state of Ohio; is a DOH license required?
