Statement of Need for: Physical Medicine and Rehabilitation Service (PM&Rs) STATEMENT OF WORK The Department of Veteran Affairs, Altoona (James E Van Zandt) VA Medical Center, has a BRAND NAME or EQUAL requirement for four (4) SCIFIT PRO2 UBE/LE Total Body Exercisers. BACKGROUND: The Altoona VA Medical Center s Physical Medicine and Rehabilitation Service (PM&Rs) is expanding it physical therapy options to its Community Based Outpatient Clinics (CBOCs) in Johnstown, State College, Dubois, as well as the Community Living Center in the main Altoona campus. The staff require Exercisers that are suitable and durable for a therapeutic environment and are designed to accommodate users with a wide range of strength and who may have limited mobility. This equipment is unique in that it works both upper body and lower body simultaneously and in a way that increases range of motion in arms, torso, and legs. SCOPE OF WORK: Vendor will be responsible for the unpacking, assembly, and testing of equipment. No remanufacturers or gray market items will be acceptable. Vendor shall be an OEM, authorized dealer, authorized distributor or authorized reseller for the proposed medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts), verified by an authorization letter or other documents from the OEM, such that the OEM s warranty and service are provided and maintained by the OEM. All software licensing, warranty, and service associated with the medical supplies, medical equipment and/or services contracts for maintenance of medical equipment shall be in accordance with the OEM terms and conditions. The delivery of gray market items to the VA in the fulfillment of an order/award constitutes a breach of contract. Accordingly, the VA reserves the right to enforce any of its contractual remedies. This includes termination of the contract or, solely at the VA s election, allowing the Vendor to replace, at no cost to the Government, any remanufactured or gray market item(s) delivered to a VA medical facility upon discovery of such items.
