Page 1 of 2 Purpose and Objectives: The intent of this Sources Sought Notice is to identify potential businesses, especially any Small Businesses, Service Disabled Veteran Owned Small Businesses (SDVOSB) or Veteran Owned Small Businesses (VOSB) offerors capable of providing LED Slit Lamp and Tonometer supplies as listed in the below Statement of Work utilizing NAICS Code 339112 (Surgical and Medical Instrument Mfg.). Responses to this Sources Sought Notice should demonstrate the firm's ability, capability and responsibility to provide the principal components of supplies listed below. Responses should include the following information: Business name, address, Point of Contact, Service-Disabled Veteran Owned Small Business info and if they are a manufacturer of the items. All information is to be submitted via e-mail at ronald.kline@va.gov. Information provided will not be returned. All responses shall be in the English Language. Responses are due by 12:00 pm (EST) on Thursday, March 18, 2021. No submissions will be accepted after this date and time. Questions can be submitted electronically to ronald.kline@va.gov. This is a Sources Sought Notice and submissions will be used for informational and planning purposes only. This notice does not constitute a formal Request for Quote (RFQ), nor is the government obligated to issue an RFQ. In addition, the Government does not intend to pay for any information provided under this notice. The Government is not obligated to notify respondents of the results of this survey either. Statement of Need VA Pittsburgh University Drive and Washington CBOC needs Haag Streit Bq900 Slit lamps with T900 Tonometers. These 2 Slit lamps are needed at the University Drive location to be able to keep up with the current patient demand. 2 of the slit lamps currently in use are end of life and do not function properly. BQ900 Slit lamps are required to replace aging and malfunctioning slit lamps. They are the primary tool used for optometrists and ophthalmologist to examine eyes and to identify and diagnose pathology. The BQ900 slit lamps are a led version that will no longer need oem specific replacement bulbs that are expensive and short lived. The old model slit lamp gears are worn and sticking and in convince doctors and do not accommodate patients range of motion. These slit lamps need to be compatible and standardized to current equipment at the VAPHS Eye Clinic Page 2 of 2 Page 2 of 2 DISCLAIMER This RFI is issued solely for information and planning purposes only and does not constitute a solicitation.
