The Wilkes-Barre VAMC is seeking SDVOSB/VOSB and any small or large company regardless of Socio-economic category that can provide the requirement as spelled out in the Statement of Need (SON) below. The veteran business must be registered in betaSAM and certified in VETBIZ (VIP). The small or large businesses must be registered in betaSAM. All Vendors interested in, and with the capability and capacity to fulfill this requirement, are required to furnish the following minimum information in reply to the sources sought: Company Name, address, Government Business Point of Contact, telephone number and email address. Company DUNS number, CAGE Code, and Tax ID numbers. Identify any current Federal Supply Schedule, GSA Contracts held, if none then state that no FSS/GSA schedules are held. Certification from the manufacturer that they have permission to provide and install any systems or software. Are you a manufacturer, dealer, or distributor? The NEUROTHERM NT-2000 LESION GENERATOR MODEL# RFG-NT-2000 device is used as a therapy for chronic pain, denervating nerves helping to lessen the pain for the patient, and possibly stopping the chance or need of surgery for the patient. This is a brand name only requirement.
