The Erie VAMC is seeking SDVOSB/VOSB and any small or large company regardless of Socio-economic category that can provide the requirement as spelled out in the Statement of Work (SOW) below. The items are brand name only. The veteran business must be registered in betaSAM and certified in VETBIZ (VIP). The small or large businesses must be registered in betaSAM. All Vendors interested in, and with the capability and capacity to fulfill this requirement, are required to furnish the following minimum information in reply to the sources sought: Company Name, address, Government Business Point of Contact, telephone number and email address. Company DUNS number, CAGE Code, and Tax ID numbers. Identify any current Federal Supply Schedule, GSA Contracts held, if none then state that no FSS/GSA schedules are held. Certification from the manufacturer that they have permission to provide and install any systems or software. Are you a manufacturer, dealer, or distributor? STATEMENT OF WORK The Department of Veterans Affairs, Erie VA Medical Center, Physical Medicine and Rehabilitation Service, has a requirement for a quantity of two (2) Aurical Real Ear and HIT systems which consist of two items each: The Aurical Probe Mic Measurement, Counseling and Simulation (PMM & C&S) Real Ear System The Aurical HIT Test Box. Requirement complies with VHA Directive 1031(1) Prescribing and Providing Eyeglasses, Contact Lenses and Hearing Aids and VHA Handbook 1170.02 VHA Audiology and Speech Language Pathology Services. Salient Features (Brand Name) Aurical (PMM & C&S) Real Ear System without Hi-Pro Compatible with OTOsuite software using Bluetooth technology Wireless Binaural measurement capability Modular system capability Frequency resolution 1/24 octave or 1/3 octave, 4 channels Intensity accuracy: +/- 3 dB Measurement intensity range: 40 130 dB Aurical HIT Test Box Compatible with OTOsuite software using Bluetooth technology Modular system capability Test level range: 40 100 dB Battery simulator measurement range: 0.5 to 40 mA Battery simulator measurement accuracy: +/- 5% Training and Installation: Training to be coordinated with a government point of contact and provided within 30 workdays after successful award. SECURITY & PRIVACY CONTROL: Contractor will be on station to provide service and therefore required to take the generalized privacy training: Privacy Training for Personnel Without Access to VA Computer Systems or Direct Access to or the Use of VA Sensitive Information Training is required upon hire and annually thereafter if the contract is in place. Contractor can self-enroll in TMS, take VA Privacy Training course # VA20939, and provide a copy of the completed training certificate to the government point of contact or COR if applicable. Alternatively, the contractor can read the text version and provide a copy of the completed training certificate to the government point of contact or COR if applicable.
