Page 3 of 3 SOURCES SOUGHT NOTICE GE MAC 5000 EKG This is a SOURCES SOUGHT ANNOUNCEMENT ONLY. It is neither a solicitation announcement nor a request for quote (RFQ) and it does not obligate the Government to award a contract. Request for a solicitation will not receive a response. The purpose of this sources sought announcement is for market research to make the appropriate acquisition decisions and to gain knowledge of potential qualified sources and their Small Business Administration (SBA) size classification in relation to North American Industry Classification System (NAICS) 33450. Background. Department of Veterans Affairs, VISN 04, Wilmington VA Medical Center, Community Based Outpatient Center (CBOC) Kent County. We currently have two GE MAC 5000 EKG machines at our main facility and in our CBOC locations. This CBOC is moving to a new location and therefore, new EKG machines are being requested as their current ones are past their life expectancy. Scope. Contractor is to provide, install and set up two (2) GE MAC 5500 HD EKG machines with mobile trolleys at the Wilmington VA Medical Center, Wilmington, DE. Specific Tasks. The contractor shall provide, install, and set up the following EKG machines: 2 GE MAC 5500 HD EKG machines 2 GE MAC 5500 HD N. America Power Cord Package 2 GE MAC 5500 HD Eng Barcode Package 2 GE MAC 5500 HD Clinical Options Pkg Crit Values ACS ACI TIPI 2 GE MAC 5500 CAM HD Acquisition Module Kit 2 GE MAC 5500 HD Mobilelink 2.4 and 5 GHZ w/ trolley bracket 2 GE MAC 5500 HD Eng Language Package 2 Modular MAC Trolley, fully assembled 2 Barcode holder kit, trolley 2 Paper Thermal VSD 155MM 8/CS 2 Silver MACtrode Plus Electrodes, 10/card, 1000/case The contractor shall provide onsite installation to include configuration for both carts. All units must communicate with the MUSE Server located at the Wilmington VAMC. Contractor will set up and install each EKG machine including wireless transmission as needed. Performance Monitoring All units purchased will be subject to an incoming inspection by Biomedical Engineering to ensure the units are functional and meet electrical safety standards as defined by the current edition of NFPA 99. Security Requirements. The Contractor will be installing and setting up medical devices which can contain protected health information. The Contractor will not routinely access this information but may have to do so in the course of installing the medical device. The Contractor will not disclose this protected health information. The C&A requirements do not apply and a Security Accreditation Package is not required. Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI). There is no GFE or GFI anticipated with this contract.Other Pertinent Information or Special Considerations. Identification of Possible Follow-on Work. There is no follow-on work anticipated. Identification of Potential Conflicts of Interest (COI). None identified. Identification of Non-Disclosure Requirements. Incidental to setting up and installing these units, the Contractor will access a work station containing ePHI. Contractor will not disclose any ePHI that may be viewed incidental to installing the purchased equipment. Packaging, Packing and Shipping Instructions. There are no special requirements. Inspection and Acceptance Criteria. As previously noted, all units purchased will be subject to an incoming inspection by Biomedical Engineering to ensure the units are functional and meet electrical safety standards as defined by the current edition of NFPA 99. Risk Control. Contractor is responsible for providing all applicable training and protective equipment to protect Contractor s employees from any hazards, including blood borne pathogens, which may be encountered in the performance of this contract. Place of Performance. All work will be performed at the VA Medical Center, 1400 Black Horse Hill Road, Coatesville, PA, 19320. Period of Performance. Contractor will have four (4) months from contract award to complete the installation. 10.Delivery Schedule. Contractor will set delivery schedule to ensure that installation can be completed within four (4) months from contract award. If possible, provide an estimate cost for this service. All responses must be received no later than December 11, 2020, 15:00 (Eastern Standard Time). Will supply the end products of a small business manufacturer or processor made in the United States and explain how your company takes ownership of the supply with any information of what part of the manufacturing process your small business entity participates in the development besides the delivery.
