THIS IS A SOURCES SOUGHT NOTICE ONLY. THIS IS NOT A REQUEST FOR PROPOSAL. The Lebanon VA Medical Center, 1700 S. Lincoln Ave, Lebanon, PA 17042 is performing market research to determine if there is a sufficient number of qualified (1) Service-Disabled Veteran Owned Small Business; (2) Veteran Owned Small Business; or (3) Small or emerging small business firms to set aside a single award contract. The Government will use responses to this notice to make to an appropriate acquisition decision. The contractor shall be responsible for providing all components of the requirement necessary to perform compounding services as needed for contingency or emergent needs for the Lebanon VAMC. Contingency compounding may be for hazardous sterile and non-hazardous compounded products which are necessary to provide infusion treatment to Veterans diagnosed with specific medical conditions. Compounding must be done at and FDA 503A and/or 503 B compliant facility and meet all state and federal laws. The contractor shall be required to pick up drugs/supplies for the Lebanon VAMC, return to the Contractor s pharmacy/facility and compound drugs as prescription details, and deliver the compounding items back to the Lebanon VAMC. The anticipated period of performance is five (5) years, with task orders to be issued as services are required. All interested firms who can meet the requirements stated above should respond, in writing, with information describing their interest, capabilities, geographic location and indicate which small business category for which they qualify to the Contract Specialist Heather Krah at heather.krah@va.gov, no later than 3:00 p.m. (Eastern Time), January 3, 2020. Solicitation 36C24420R0028 Pharmacy IV Compounding Emergency Services has been assigned and should be referenced on all correspondence regarding this announcement. The NAICS Code 446110 is applicable to this acquisition, with a Business Size Standard of $30.0 M. Interested firms are reminded that in accordance with FAR 4.12, prospective contractors shall complete electronic annual representations and certifications in conjunction with FAR 4.11 required registration in the Systems for Award Management (SAM) database prior to award of a contract.
