SEE ATTACHED JUSTIFICATION AND NOTICE OF INTENT TO SOLE SOURCE INTENT TO SOLE SOURCE The Lebanon VAMC HUB VISN-04 intends to negotiate a sole source contract under the authority of FAR 6.302-1 (Only one responsible source and no other supplies or services will satisfy agency requirements) for In-Home Mattress Rental / Purchase with Columbia Ancillary Services, Inc. of 978 Spring St, Houtzdale, PA 16651-1715. The Justification and Approval for this action will be included as a part of the award notice. Simplified Acquisition Procedures will be used and the resulting contract will be a firm fixed price contract. The NAICS Code is 339113 and the Size Standard is 500 employees.
