The Lebanon VAMC is seeking SDVOSB/VOSB and any small or large company regardless of Socio-economic category that can provide the requirement as spelled out in the Statement of Work (SOW) below. The veteran business must be registered in SAM and certified in VETBIZ (VIP). The small or large businesses must be registered in SAM. All Vendors interested in, and with the capability and capacity to fulfill this requirement, are required to furnish the following minimum information in reply to the sources sought: Company Name, address, Government Business Point of Contact, telephone number and email address. Company DUNS number, CAGE Code, and Tax ID numbers. Identify any current Federal Supply Schedule, GSA Contracts held, if none then state that no FSS/GSA schedules are held. Certification from the manufacturer that they have given you permission to provide and install any systems or software. Responses shall be submitted electronically, via email to Donna Toliver. This requirement is for the purchase of multiple InFab Revolution Radiology Protection Garments (measured and fitted for size) Vests/Skirts, Gowns, and Thyroid Collars which provide protection from radiologic exposure. The request is for Sole Source Firm Fixed Price requirement. See the Statement of Work (SOW) and items below. If expressing an interest in this requirement you will need to provide a letter from the manufacturer that you are authorized dealer or distributor. Revolution X-Ray Protection Gowns/Vests & Skirts/Thyroid Collars Statement of Need Background: The Lebanon VA Medical Center (LVAMC) has a requirement for multiple InFab Revolution Radiology Protection Garments (measured and fitted for size) Vests/Skirts, Gowns, and Thyroid Collars which provide protection from Radiologic Exposure. Scope of Work: The protective garments mentioned above are needed in the OR setting to protect staff from Radiologic exposure when utilizing imaging devices such as a C-Arm, etc. Salient Characteristics of the Protective X-Ray Garments: Shall provide 0.50mm front, and 0.25mm back protection Core Material shall be KI-KI Armor Bi-Layer Lead Free Fabric used in production of the garments must be Revolution Fabric Lightweight Waterproof Abrasion Resistant Flame Resistant Bacterial resistant See Below for KIARMOR LEAD FREE SPECIFICATIONS: NOTE: No Personalization or Embroidered Embellishment Permitted/Wanted Additional Information: The InFab Revolution Garments are requested due to their exclusive fabric and lead-free design (KIARMOR Bi-Layer Lead Free). InFab is the only manufacturer with an FDA Registration for lead free aprons. These garments are also the most lightweight and offer the highest protection levels of any garments on the market. The providers receiving the garments, have been specifically measured for the garments, which is a high priority factor in the functionality and safety of the garment to the provider. Delivery: Delivery shall be within thirty (30) days from the time of award. The Radiology Protective Garments and accessories shall be delivered to the Lebanon VAMC located at 1700 S. Lincoln Ave., Lebanon, PA 17042; Attn; Warehouse. BioMedical Engineering will need to perform a safety check and enter the equipment in to the system for annual Preventive Maintenance (PM) checks.
