The Contractor shall furnish all services in accordance with the performance work statement and schedule identified in the solicitation to provide services necessary to perform On-Call Vascular Surgeon Services to eligible beneficiaries of the Department of Veterans Affairs Medical Center. The subsequent contract will be performed for the Department of Veterans Affairs, Lebanon Veterans Affairs Medical Center, 1700 South Lincoln Avenue Lebanon, PA 17042. As a result of the solicitation, the Government intends to award one (1) 5 - year contract IDIQ, made to one offeror who meets the response requirements listed in the forth coming solicitation. The applicable NAICS code is 621111 with a size standard of $12.0 million in average annual receipts. Award will be made to the offeror whose quote is responsive and conforming to the RFQ using Lowest Price Technically Acceptable procedures (LPTA).
