This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The combined synopsis solicitation number is 36C24420Q1028. It is issued as a Request for Quote (RFQ). The government anticipates awarding one firm-fixed price Indefinite Delivery Indefinite Quantity (IDIQ) contract resulting from this solicitation. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2020-07 (08/31/2020) and can be found at http://www.acquisition.gov/far/index.html. The North American Industrial Classification System (NAICS) code for this procurement is 339116 with a small business size standard of 500 employees. This solicitation is unrestricted. This solicitation requires registration with the System for Award Management (SAM) prior to award, pursuant to applicable regulations and guidelines. Registration information is at www.sam.gov. Confirmation of SAM registration will be validated prior to awarding a contract. The Department of Veterans Affairs Medical Center (VAMC) in Erie, Pennsylvania requires a contract for offsite dental services for patients at the Erie VA Medical Center (EVAMC), which is located at 135 East 38 Street, Erie, PA 16504. The Contractor shall provide all labor, supervision and all other resources required to deliver dental lab services to include work by qualified dental lab personnel, materials, repairs, and finished products, for and pertaining to the items listed in the schedule, in accordance with the contract documents. The contracted dental lab will fulfill the standards set by our current responsiveness to the Veterans of the Erie VAMC population as determined by current standards of customer satisfaction or established wait times for dental products. The contractor must be capable of meeting and maintaining the Turn-Around Times (TAT) stated in the Statement of Work. The Erie VAMC intends to award a contract, for a base period beginning 10/14/20 9/30/2021 with four, one-year option periods. Technical Capability (Technical Proposal & Staffing Approach): Under no circumstances shall pricing information be included in the Technical Capability. Technical Proposal: Quoters shall submit the following information: a. A detailed description of contractor s technical proposal that demonstrates your ability to perform the work described in the Statement of Work (SOW). Quoters are required to address all the SPECIFIC MANDATORY TASKS AND ASSOCIATED DELIVERABLES in their proposal for items numbered 1-10 in the Statement of Work (i.e., Maxillary Denture and Mandibular Denture, etc.). In addition, quoters are required to be very specific in outlining their ability to provide a courier service to meet the requested TATs described in the Statement of Work especially in regard to basic denture repairs and relines. b. Copy of dental facility s licenses, permits, and certification required by law. c. Contingency plan to be utilized in the event equipment or supplies are unavailable for scheduled dental work in accordance with the terms and conditions of the resulting contract. d. List of key personnel to include qualifications, training, resume, license/certification. e. Written confirmation that the contractor s courier service will not be subcontracted to a commercial carrier e.g. UPS, FedEx, Priority Mail, USPS, DHL, etc. f. A detailed description of how the contractor shall meet the 24-hour turn-around time and other proposed delivery times. Staffing Approach: Quoters shall submit the following information: a. Staffing Approach The contractor shall submit a sound staffing approach as reflected in a staffing table, which identifies the necessary subcontractor given the Offeror s approach to performing the SOW e.g. courier service and/or other subcontracted work. The proposed staffing approach shall be reflected in a table which correlates the name of the subcontractor(s), work to be performed in accordance with the SOW, identifies the percentage of subcontracted work, and the socioeconomic category of the subcontractor. Past Performance: Quoters shall provide up to three (3) references, preferably of Government work, performed in the last five (5) years for the prime contractor and/or subcontractor(s). Past performance information shall be recent (performance within the last five years) and relevant to this scope of work. All past performance references shall include name of client, customer point of contact, customer telephone number, contract number, award amount, project/service location, and other relevant information. The apparent awardees past performance will be evaluated as part of the contractor responsibility determination to ensure the presence of an acceptable performance record in accordance with FAR 9.104-1. In addition, if Quoters are using a subcontractor and that subcontractor is for courier services, the Quoter shall only submit one (1) past performance reference on the subcontractor. The Government reserves the right to seek past performance information (prime and/or subcontractor(s)) from any sources not included in your past performance references and available to the Government to include, but not limited to, CPARS or other databases; interviews with Program Managers, customers, and Contracting Officers. 3. FAR 52.212-2 Evaluation Commercial Items (OCT 2014) applies to this acquisition: (a) The Government will award a contract resulting from this solicitation to the responsible Quoter that represents the best benefit to the Government at a price that can be determined reasonable. The Government will evaluate technical capability, past performance and price. (b) A written notice of award or acceptance of a quote, mailed or otherwise furnished to the successful Quoter within the time for acceptance specified in the quote, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept a quote (or part of a quote), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. Technical Capability, Past Performance and Price. The Government will perform an evaluation using a comparative evaluation. The Government will compare quotes to one another to select the contractor that best benefits the Government by fulfilling the requirement. Comparative evaluation will be conducted in accordance with FAR 13.106-2(b)(3). The Contracting Officer will award one, firm fixed price IDIQ contract resulting from this RFQ to the responsible Quoter whose quote conforming to the RFQ is most advantageous to the Government. FAR 52.216-18 ORDERING (OCT 1995) (a) Any supplies and services to be furnished under this contract shall be ordered by issuance of delivery orders or task orders by the individuals or activities designated in the Schedule. Such orders may be issued from October 14, 2020 through September 30, 2025. (b) All delivery orders or task orders are subject to the terms and conditions of this contract. In the event of conflict between a delivery order or task order and this contract, the contract shall control. (c) If mailed, a delivery order or task order is considered "issued" when the Government deposits the order in the mail. Orders may be issued orally, by facsimile, or by electronic commerce methods only if authorized in the Schedule. (End of Clause) FAR 52.216-19 Order Limitations (OCT 1995) (a) Minimum order. When the Government requires supplies or services covered by this contract in an amount of less than $50.00, the Government is not obligated to purchase, nor is the Contractor obligated to furnish, those supplies or services under the contract. (b) Maximum order. The Contractor is not obligated to honor (1) Any order for a single item in excess of $25,000.00; (2) Any order for a combination of items in excess of $50,000.00; or (3) A series of orders from the same ordering office within 90 days that together call for quantities exceeding the limitation in paragraph (b)(1) or (2) of this section. (c) If this is a requirements contract (i.e., includes the Requirements clause at subsection 52.216-21 of the Federal Acquisition Regulation (FAR)), the Government is not required to order a part of any one requirement from the Contractor if that requirement exceeds the maximum-order limitations in paragraph (b) of this section. (d) Notwithstanding paragraphs (b) and (c) of this section, the Contractor shall honor any order exceeding the maximum order limitations in paragraph (b), unless that order (or orders) is returned to the ordering office within 5 days after issuance, with written notice stating the Contractor's intent not to ship the item (or items) called for and the reasons. Upon receiving this notice, the Government may acquire the supplies or services from another source. (End of Clause) 52.216-22 Indefinite Quantity (OCT 1995) (a) This is an indefinite-quantity contract for the supplies or services specified, and effective for the period stated, in the Schedule. The quantities of supplies and services specified in the Schedule are estimates only and are not purchased by this contract. (b) Delivery or performance shall be made only as authorized by orders issued in accordance with the Ordering clause. The Contractor shall furnish to the Government, when and if ordered, the supplies or services specified in the Schedule up to and including the quantity designated in the Schedule as the "maximum." The Government shall order at least the quantity of supplies or services designated in the Schedule as the "minimum." (c) Except for any limitations on quantities in the Order Limitations clause or in the Schedule, there is no limit on the number of orders that may be issued. The Government may issue orders requiring delivery to multiple destinations or performance at multiple locations. (d) Any order issued during the effective period of this contract and not completed within that period shall be completed by the Contractor within the time specified in the order. The contract shall govern the Contractor's and Government's rights and obligations with respect to that order to the same extent as if the order were completed during the contract's effective period; provided, that the Contractor shall not be required to make any deliveries under this contract after the end of the contracted period of performance. (End of Clause) FAR 52.217-9 OPTION TO EXTEND THE TERM OF THE CONTRACT (MAR 2000) (a) The Government may extend the term of this contract by written notice to the Contractor within 15 days; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 30 days before the contract expires. The preliminary notice does not commit the Government to an extension. (b) If the Government exercises this option, the extended contract shall be considered to include this option clause. (c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed five (5) years.
